摘  要:目的 研究MR扩散峰度成像(DKI)和定量磁敏感图(QSM)在帕金森病(PD)临床诊断中的应用价值。方法 20例PD患者和20例健康对照组(HC组)分别进行DKI及QSM序列扫描,利用DKE和MEDI软件得到各参数图,由2名至少具有5年工作经验的放射科医师手动描绘黑质、红核、尾状核头、壳核、丘脑的感兴趣区(ROI)范围,并测得DKI各参数[平均扩散峰度(MK)、横向峰度(AK)、径向峰度(RK)、平均扩散率(MD)、各向异性分数(FA)]及QSM(磁化率值)。利用t检验对比PD组及HC组组内和组间各核团各参数值的差异性,并将2组图像各部位各参数间进行相关性分析。结果 PD组(DKI3b)及HC组(DKI3b、DKI5b)各组内各核团DKI各参数值均无侧别差异(P〉0.05);DKI3b与DKI5b 2组图像比较各部位各参数均无统计学差异(P〉0.05);与HC组相比,PD组黑质、丘脑MK均升高且有统计学差异(P〈0.05),PD组黑质、尾状核头MD均升高且有统计学差异(P〈0.05);PD组黑质、红核、尾状核头、壳核磁化率值均减少且有统计学意义(P〈0.05);黑质、红核、尾状核头、壳核、丘脑的磁化率值与对应的DKI各参数均未见明显相关性(P〉0.05)。结论 DKI参数可以分析评估PD患者脑深部核团微观结构的变化;QSM可以定量评估脑深部核团的铁沉积含量,MR DKI和QSM对PD的诊断具有一定意义。Objective To evaluate the clinical value of diffusion kurtosis imaging (DKI) and quantitative susceptibility mapping (QSM) in the diagnosis of Parkinson' s disease (PD).Methods 20 PD patients and 20 healthy subjects [healthy controls(HC) ] were recruited in this study and scanned by using DKI and QSM techniques.Firstly,DKI-and QSM-derived parameters including mean kurtosis (MK) , axial kurtosis ( AK ) , radial kurtosis ( RK ), mean diffusivity ( MD), fractional anisotropy ( FA ) and susceptibility mapping were obtained by using DKE and MEDI toolbox.Then, the ROIs including the substantia nigra, red nucleus,head of caudate nucleus, putamen and thalamus were manually drawn on each parameter mapping images by two radiologists with more than 5 years clinical experience.The values within the drawn ROIs on DKI-derived (MK, AK, RK, MD, FA) and QSM-derived ( susceptibility values) were extracted. Finally, two-sample students' t-test were performed to find the difference between PD and HC groups on each parameter and ROIs.The correlation analysis between each DKI-derived parameters and susceptibility values in 20 PD patients was carried out. Results There was no any difference between the bilateral ROIs in the all nucleis of DKI parameters (P〉0.05) in neither group. There was no significant difference in DKI3b and DKIsB PD groups of for any DKI-derived parameters (P〉0.05 ).Compared with HC group, MK in PD group increased within both the substantia nigra and thalamus (P〈0.05}. MD increased within both the substantia nigra and head of caudate nucleus in PD group (P〈0.05}.The susceptibility values within the substantia nigra,red nucleus,head of caudate nucleus and putamen decreased in PD group(P〈0.05). The susceptibility values of the substantia nigra, red nucleus, head of caudate nucleus, putamen and thalamus showed no correlation (P〉0.05} with any DKI-derived parameters.
