摘　　要：目的探讨原发性中枢神经系统淋巴瘤的临床表现特点及磁共振成像(MRI)诊断与鉴别诊断价值。方法回顾性分析经手术病理证实的10例(男6例,女4例)原发性中枢神经系统淋巴瘤患者的临床及影像资料,重点分析MRI特点及手术病理特征。结果 10例患者均行磁共振平扫与增强检查。T_1WI像7例呈低信号,3例呈等信号;T_2WI像7例呈稍高信号,3例呈等信号;T2FLAIR像6例高信号,4例稍高信号;DWI像8例呈明显高信号;2例稍高信号。静脉注入造影剂钆喷酸葡胺(Gd-DTPA)后,7例明显强化,3例不均匀强化。结论原发性中枢神经系统淋巴瘤MRI影像表现具有特征性,对病变诊断及鉴别诊断有重要价值。【Objective】To investigate the clinical features and the value of MRI in diagnosing the primary central nervous system lymphoma.【Methods】We analyzed the clinical and imaging data of 10 cases（6 cases of male, 4 cases of female） of primary central nervous system lymphoma retrospectively, which were confirmed by surgery and pathology. We mainly analyzed MRI features and surgical pathologic features.【Result】All the 10 patients underwent MR plain scan and enhanced examination. On T_1WI, 7 cases showed low signal intensity, 3 cases showed iso-signal intensity. On T_2WI, 7 cases showed slightly high signal intensity, 3 cases showed iso-signal intensity. On FLAIR, there were 6 cases of hyperintensity, 4 cases of slightly hyperintensity. On DWI, 8 cases showed obviously hyperintensity, 2 cases showed slightly high signal intensity. After the injection of Gd-DTPA, 7 patients showed obvious enhancement, 3 patients showed inhomogeneous enhancement.【Conclusion】The MRI manifestation of the primary central nervous system lymphoma has its characteristic, which is of great value for the diagnosis of the disease.
