摘 要:目的探讨放射性^(125)I粒子组织间植入治疗不可手术切除软组织肉瘤的疗效和安全性。方法收集2008年11月至2017年3月就诊河北省人民医院行放射性^(125)I粒子植入治疗的9例软组织肉瘤(共10个病灶)患者的临床资料。肿瘤最大径1.5~20 cm,植入粒子18~180颗,单颗粒子活度0.3~0.8 mCi。周边剂量92~169 Gy,观察不可手术切除患者单纯采用放射性^(125)I粒子植入术后6个月的局控率、体力状况(performance status,PS)、疼痛缓解情况及并发症。结果放射性粒子植入术后6个月肿瘤病灶局控率为90%(9/10),PS评分由粒子术前1.6±0.7降为1.1±0.6,P=0.046;疼痛评分由术前平均3.8分降为1.3分,P=0.026。1例粒子植人后出现皮肤溃疡,经换药后愈合,余患者未见并发症。结论放射性^(125)I粒子植入治疗软组织肉瘤,尤其是不可手术切除患者安全有效。Objective To evaluate the curative effect and safety of radioactive (125)I seed interstitial implantation in treating unresectable soft tissue sarcomas. Methods The clinical data of 9 patients with soft tissue sarcoma(10 lesions in total), who were admitted to Hebei Provincial People's Hospital, to receive radioactive (125)I seed interstitial implantation therapy during the period from November 2008 to March 2017,were retrospectively analyzed. The maximum diameters of the lesions were 1.5-20 cm. The implanted radioactive particles were 18-180 seeds, the peripheral dose was 92-169 Gy, and the single seed activity was 0.3-0.8 mCi. The local control rate, performance status(PS), pain relief degree and incidence of complications were determined at 6 months after simple radioactive (125)I seed interstitial implantation therapy.Results Six months after radioactive (125)I seed interstitial implantation, the local control rate of tumor lesions was 90%(9/10), PS score decreased from preoperative(1.6±0.7) points to postoperative(1.1±0.6) points,and the difference in the change of PS score was statistically significant(P=0.046). The pain score decreased from preoperative mean 3.8 points to postoperative mean 1.3 points, and the difference was statistically significant(P=0.026). Skin ulceration after seed implantation occurred in one patient, which was cured after dressing change. No other complications were observed. Conclusion For the treatment of soft tissue sarcomas, especially inoperable ones, radioactive (125)I seed implantation is safe and effective
