氟斑牙冷光漂白与家庭漂白后结合黏接剂应用的疗效比较

摘　　要：目的比较氟斑牙冷光漂白与家庭漂白后结合黏接剂应用减少术后再着色的临床疗效。方法将中度着色氟斑牙患者104例,随机分成A、B、C、D 4组各26例,A组冷光漂白+釉质黏接剂,B组冷光漂白,C组家庭漂白+釉质黏接剂,D组家庭漂白。治疗结束后比较各组色阶变化、牙齿敏感性及牙周组织刺激情况。结果治疗结束后3个月时4组总有效率均为100.0%。治疗结束后12个月时,A组总有效率高于B组,C组总有效率高于D组,差异有统计学意义(P<0.05)。A组与C组术后即刻和术后3,7,30d牙齿均无敏感发生;B组术后即刻和术后3d牙齿敏感率高于A组,D组术后即刻和术后3d牙齿敏感率高于C组,差异均有统计学意义(P<0.05);A组与B组、C组与D组术后7,30d牙齿敏感率差异均无统计学意义(P>0.05)。4组中度着色氟斑牙漂白后牙龈指数差异无统计学意义(P>0.05)。结论中度氟斑牙冷光漂白与家庭漂白后结合釉质黏接剂应用,可有效减低再着色率,降低氟斑牙漂白后空气刺激的敏感性。Objective To compare the re-coloring effect of cold light whitening and inhome bleaching for dental fluorosis by the enamel adhesive binding.Methods The 104 patients with moderate staining fluorosis were randomly divided into four groups：A,B,C and D,with 26 in each group,goup A cold light whitening＋enamel adhesive,group B cold light whitening,group C in-home bleaching ＋ enamel adhesive,group D in-home bleaching.After treatment,color change,tooth sensitivity and periodontal tissue stimulation were compared.Results After3 months,the total effective rate of the four groups was 100.0%.After 12 months,group A is higher than group B,group C is higher than group D,the difference is statistically significant（P〈0.05）.There was no tooth sensitivity in group A and group C immediately after bleaching,3 days after bleaching,7 days,and 30 days.The sensitivity of teeth was higher in group B than in group A immediately after bleaching and 3 days after bleaching.The sensitivity of teeth was higher in group D than in group C immediately after bleaching and 3 days after bleaching,the difference is statistically significant（P〈0.05）.There was no significant difference in tooth sensitivity rate 7 days and 30 days after bleaching in groups A and B,C and D（P〉0.05）.There was no significant difference in gingival index in 4 groups after moderate fluorosis bleaching（P〉0.05）.Conclusion The use of enamel adhesive can effectively reduce the chance of re-coloration and the sensitivity of air stimulation after moderate dental fluorosis by cold light whitening and family whitening.
