摘　　要：目的探讨河北汉族成年人群中白细胞介素4(interleukin-4,IL-4)rs2070874基因型及其基因频率与幽门螺杆菌(H.pylori)感染、胃癌临床易感性的关系。方法收集本院就诊患者分为3组:H.pylori感染组105例,未感染组103例,胃癌组103例。留取患者外周静脉血,采用直接测序法对IL-4rs2070874进行基因型测定。结果IL-4rs2070874基因型在H.pylori感染组以TT型为主(55.24%),胃癌组以TC型为主(55.34%),未感染组以TT型为主(44.66%);胃癌组TC基因型高于H.pylori感染组和未感染组,基因型分布差异有统计学意义(P<0.05),而H.pylori感染组与未感染组基因型分布差异无统计学意义。3组人群该位点均以T基因为高表达基因,3组人群T、C等位基因频率差异无统计学意义(P>0.05)。该位点C为低频率表达基因,表达频率为0.321 5。结论 IL-4rs2070874基因多态性与胃癌易感性有关,而与H.pylori感染无关,该位点TC基因型可能增加胃癌风险。Objective To evaluate the relationship of interleukin-4（IL-4）rs2070874 gene polymorphism with H.pylori infection and susceptibility to gastric cancer in different groups of the Han nationality adults.Methods A total of 311 subjects were recruited and divided into three groups：105 H.pylori positive,103 gastric cancer people,and 103 H.pylori negative.The peripheral venous bloods were extracted and direct sequencing method were used for IL-4 rs2070874 gene polymorphism analysis.Results The main genotype of IL-4 rs2070874 in H.pylori infection group was TT type,accounting for 55.24% of the total,TC type was the main genotype in gastric cancer group,accounting for 55.34% of the total,and TT type was the main genotype in non-infection group,accounting for 44.66% of the total.TC genotype in gastric cancer group was higher than that in H.pylori infection group and non-infection group,and the genotype distribution had significant difference（P〈0.01）.But no significant difference was found between H.pylori positive and negative group.T was the high expression gene of this locus and no different frequency was founded in the three groups（P〉0.05）.The C frequency of IL-4 rs2070874 in Han nationality adults was 0.3215.Conclusion IL-4 rs2070874 gene polymorphism may be associated with gastric cancer susceptibility,but not with H.pylori infection,TC gene may increase the risk of gastric cancer.
