TS在Ⅱ/Ⅲ期SiewertⅡ型食管胃结合部腺癌组织中的表达及与预后的关系

摘　　要：目的:分析Ⅱ、Ⅲ期SiewertⅡ型食管胃结合部腺癌组织中胸腺嘧啶合成酶(TS)的表达水平,进一步探讨其与以5-FU为基础的化疗疗效及预后的相关性。方法:收集36例术后接受5-FU为基础化疗的食管胃结合部腺癌患者,采用RT-PCR方法分析TS mRNA表达水平及与化疗疗效和预后的相关性。结果:肿瘤组织的TS mRNA表达水平(0.820±0.185)明显高于癌旁正常组织(0.468±0.129,P<0.001),在发生淋巴结转移患者中TS mRNA表达(0.960±0.995)明显高于未发生淋巴结转移的患者(0.732±0.172,P=0.001),组织学分化程度越差的患者TS mRNA表达水平越高(P=0.028),TS mRNA高表达的患者生存率均偏低(P<0.05)。结论:Ⅱ/Ⅲ期SiewertⅡ型食管胃结合部腺癌患者的TS mRNA水平升高提示患者预后不良。TS mRNA表达情况对临床上制定个体化的化疗方案具有一定的指导价值。Objective： To investigate the correlation between the expression levels of thymidylate synthase（TS）, clinical outcome in patients with adenocar-cinoma of the esophagogastric junction receiving fluorouracil-based chemotherapy. To investigate whether TS mRNA was a new target of adenocarcinoma of the esophagogastric junction（ AEG） targeted therapy and the basis of molecular typing. Methods： 36/48 patients with adenocar-cinoma of the esophagogastric junction who received postoperative fluorouracil-based chemotherapy were enrolled in the study. Real-time quantitative PCR（RT-PCR） analysis was used to detect the expression levels of TS. Morever the correlation between the expression levels and therapeutic effects of chemotherapy as well as patient＇ s prognosis was analyzed. Results： The expression levels of TS mRNA in tumor tissues were significantly higher than those in normal tissue（P〈0.001）. The expression levels of TS mRNA in lymphatic metastasis were significantly higher than those in normal tissue（P=0.001）. The high expressions of TS mRNA were risk factors influncing adenocarcinoma of the esophagogastric junction survival rate. Conclusion： The high expressions of TS with poor prognosis in patients with adenocarcinoma of the esophagogastric junction,thus,the expressions of TS has certain instructive value for instituting individualized chemotherapy scheme in clinical practice.
