摘 要:目的:基于动物模型探讨骨复生对激素性股骨头坏死大鼠骨保护素(OPG),受体活化因子(RANK)信号通路的影响。方法:将80只大鼠随机分为正常组、模型组、中药组、西药组4组,每组各20只。除正常组外其余各组连续2d中每日经尾静脉注射40μg/kg的脂多糖,之后每隔3d臀部肌肉(所有动物统一从右侧臀大肌以45°角匀速注射)注射地塞米松25mg/kg,连续注射6周。观察造模前后以及治疗前后的股骨头X线变化、组织病理学改变以及OPG,NF-κB基因表达情况。结果:1)中药组、西药组双侧股骨头X线结果与正常组最为接近,股骨头大小形态正常,骨密度均匀,骨小梁清晰,但西药组仍有少部分密度不均匀;模型组股骨头密度不均,但股骨头仍完整,偶见透亮囊状阴影。2)正常组细胞结构清晰,空骨陷窝较少见;中药组可见骨细胞大部分正常,空骨陷窝较少;西药组变化大致与中药组接近,但空骨陷窝较中药组多;模型组细胞结构紊乱、细胞萎缩、细胞集聚固缩,空骨陷窝明显存在。3)中药组和西药组分别与模型组比较,中药组和西药组骨组织中RANK的表达均下调,且中药组下调更明显,而中药组和西药组骨组织中OPG表达均上调,差异有统计学意义(P<0.05);中药组和西药组与正常组比较,骨组织中OPG,RANKL及RANK的表达差异无统计学意义(P>0.05);中药组与西药组比较,骨组织中OPG的表达差异有统计学意义(P<0.05)。结论:骨复生可明显上调OPG的表达,下调RANK的表达,增加骨细胞形成,促进骨量上升,减少骨钙流失,促进骨质成熟,改善股骨头局部缺血缺氧环境,从而有效干预激素性股骨头坏死的形成。Objective: The effects of Gufusheng on osteoprotegerin (OPG) and receptor activator of nuclear factor-kB (RANK) signaling pathways in rats with steroid-induced femoral head necrosis were investigated based on animal models.Methods: Eighty rats were randomly divided into normal group, model group, Chinese medicine group and western medicine group, with 20 rats in each group. Except for the normal group, the other groups were injected with 40 μg/kg lipopolysaecharide via caudal vein daily for 2 d, then dexam- ethasone 25 mg/kg was injected into the gluteal muscles ev- ery 3 d (all animals were injected uniformly from the right gluteal muscles at 45°) for 6 weeks. The X-ray changes, his- topathological changes and the expression of OPG and NF- kB genes were observed before and after modeling and treat- ment. Results: 1)The X-ray results of the bilateral femoralheads in the Chinese medicine group and the western medicine group were the closest to the normal group. The size and shape of the femoral head were normal, the bone density was uniform, and the trabecular bone was clear, but the western medicine group still had a small density unevenness. The density of femoral head in the model group was not uniform, but the femoral head was still intact, occasionally with bright cystic shadow. 2)The cell structure of the normal group was clear, and the empty bone lacuna was less common,the Chinese medicine group showed that most of the bone cells were normal, and the empty bone lacuna was less ;the change of the western medicine group was similar to that of the tradition- al Chinese medicine group, but the empty bone lacuna was more than the traditional Chinese medicine group;The model group had cell structure disorder, cell atrophy, cell agglomeration and pyknosis, and the number of empty bone sag was obvious. 3)Compared with the model group, the expressions of RANK in the Chinese medicine group and the western medicine group were down-regulated, and the traditional Chinese medicine was used.
