美金刚替换多奈哌齐治疗中度阿尔茨海默病的临床疗效及安全性

摘　　要：目的探讨美金刚替换多奈哌齐治疗中度阿尔茨海默病(AD)的临床疗效及安全性。方法选取南京医科大学附属无锡精神卫生中心老年精神科2013—2016年收治的AD患者60例,采用随机数字表法分为对照组和研究组,各30例。对照组患者继续服用多奈哌齐治疗,研究组患者改服美金刚治疗。比较两组患者替换前及替换后第12、24周简易智能状态检查量表(MMSE)、AD行为病理评定量表(BEHAVE-AD)、日常生活能力量表(ADL)评分,并观察两组患者不良反应发生情况。结果替换前及替换12周后,两组患者MMSE评分比较,差异无统计学意义(P>0.05);替换24周后,研究组患者MMSE评分高于对照组(P<0.05)。替换前,两组患者BEHAVE-AD评分、ADL评分比较,差异无统计学意义(P>0.05);替换12、24周后,研究组患者BEHAVE-AD评分、ADL评分低于对照组(P<0.05)。两组患者均未见严重不良反应发生。结论美金刚替换多奈哌齐治疗中度AD患者的临床疗效确切,其可更有效改善患者的认知功能、精神行为症状及日常生活能力,且安全性好。Objective To evaluate the clinical effect and safety of memantine replacement of donepezil in the treatment of moderate Alzheimer＇s disease（AD）.Methods A total of 60 cases of AD patients were selected from 2013 to 2016 years in Wuxi Mental Health Center Affiliated to Nanjing Medical University,which were divided into control group and study group according to random number table method,30 cases in each group.The control group was given donepezil treatment,study group was given memantine replacement of donepezil.The MMSE scores,BEHAVE-AD scores,ADL scores before replacement and after replacement of 12,24 weeks were compared between the two groups,and the incidence of adverse reactions were observed.Results Before replacement and after replacement of 12 weeks,no significant differences of MMSE scores was found between the two groups（P0.05）;after replacement of 24 weeks,the MMSE scores in study group were higher than control group（P0.05）.Before replacement,no significant differences of BEHAVE-AD scores or ADL scores was found between the two groups（P0.05）;after replacement of 12,24 weeks,the BEHAVE-AD scores,ADL scores in study group were lower than control group（P0.05）.There was no serious adverse reactions happened.Conclusion Memantine replacement of donepezil have an exact clinical effect in the treatment of AD,it can more effectively improve cognitive function,symptoms of mental behavior and daily life of patients,and with good safey.
