脑梗死急性期血清let-7f和IL-6水平及其与病情和神经功能缺损程度的相关性

摘　　要：目的探讨脑梗死急性期患者血清let-7f mRNA表达水平和白细胞介素(IL)-6水平及其与病情和神经功能缺损程度的相关性。方法选择2013年3月至2015年9月神经内科确诊的急性脑梗死患者124例为脑梗死组,选择同期门诊健康体检者95例为对照组;对脑梗死组患者按照不同发病时间、不同神经功能缺损程度[美国国立卫生研究院卒中量表(NIHSS评分)]分组,采用实时荧光定量聚合酶链反应检测血清let-7f mRNA表达,采用酶标仪测定血清IL-6水平。绘制两组ROC曲线并比较let-7f mRNA及IL-6诊断脑梗死的灵敏度、特异度。结果脑梗死组卒中史、高血压病、高胆固醇血症、高甘油三酯血症发生率高于对照组(P均<0. 01)。NIHSS评分5～19分组高血压病、高胆固醇症、高甘油三酯血症所占比例高于NIHSS评分0～4分组(P均<0. 01)。脑梗死组IL-6水平高于对照组,let-7f mRNA水平低于对照组(P均<0. 01)。NIHSS评分5～19分组IL-6水平高于0～4分组,let-7f mRNA水平低于0～4分组(P均<0. 01)。IL-6 ROC曲线下面积(AUC)为0. 839(95%CI:0. 765～0. 985),let-7f mRNA AUC为0. 777(95%CI:0. 587～0. 903),两者AUC相近(P> 0. 05),两者诊断脑梗死的灵敏度、特异度相近(P> 0. 05)。结论脑梗死急性期患者血清let-7f mRNA表达水平降低,IL-6升高,其水平与病情和神经功能缺损程度有一定相关性;let-7f、IL-6诊断脑梗死具有较高的灵敏度、特异度,可作为判定急性脑梗死的生物学标志物。Objective To investigate the levels of serum let-7f mRNA expression and Interleukin-6（IL-6） and their associations with the state of illness and the degree of neural function deficient in patients with acute cerebral infarction.Methods A total of 124 patients with acute cerebral infarction diagnosed by neurology department from March 2013 to September 2015 were served as research objects（cerebral infarction group）,and 95 healthy subjects from outpatient physical examination at the same period were served as control group. The patients in cerebral infarction group were regrouped according to different onset time and different scores of National Institute of Health Sroke Scale（NIHSS）. Realtime fluorescence quantitative polymerase chain reaction was used to detect the expression level of serum let-7f mRNA.Multiskan FC enzyme labeling apparatus was used to detect the level of serum IL-6. The receiver operating characteristic curve（ROC curve） was drawn,and sensitivity and specificity of IL-6 and let-7f mRNA for diagnosing cerebral infarction were compared. Results The proportions of stroke history,hypertension,hypercholesterolemia,hypertriglyceridemia in cerebral infarction group were significantly higher than those in control group（all P〈0. 01）. The proportions of hypertension,hypercholesterolemia,hypertriglyceridemia in group NIHSS 5 to 19 scores were significantly higher than those in group NIHSS 0 to 4 scores（all P〈0. 01）. Compared with control group,the level of serum IL-6 was significantly higher and the expression level of serum let-7f mRNA was significantly lower in cerebral infarction group（all P〈0. 01）. Compared with group NIHSS 0 to 4 scores,the level of serum IL-6 was significantly higher and the expression level of serum let-7f mRNA was significantly lower in group NIHSS 5 to 19 scores（all P〈0. 01）. The areas under the ROC curve（AUCs） for IL-6[0. 839（95% CI：0. 765-0. 985）]and let-7f mRNA[0. 777（95% CI：0. 587-0. 903）]were similar（P〉0. 05）,and
