预防性用药对一次性根管治疗后疼痛的疗效研究

摘　　要：目的:评价预先使用镇痛药物对不可逆性牙髓炎及牙髓坏死患者根管治疗术后疼痛的缓解效果。方法:纳入2017-02-01～2018-02-28我院口腔科就诊的患者90例(其中46例为不可逆性牙髓炎,44例为牙髓坏死),随机分为实验组(泼尼松组)和对照组(安慰剂组),每组45例,两组均于术前30 min用药后行一次性根管治疗,并通过疼痛视觉评分量表(visual analogue scale,VAS)评价两组患者在术后即刻、6h、12h、24h、48h的疼痛程度。并于术后24h临床评定患者疼痛率及舒适度。结果:两组术后各观察时间点的VAS分值比较,差异有统计学意义(P<0.05);术后24h实验组疼痛发生率明显低于对照组,差异有统计学意义(P<0.05);且实验组的简化舒适状况量表(general comfort questionnaire,GCQ)各维度评分均高于对照组,差异有统计学意义(P<0.05)。结论:根管治疗前预先给予一定剂量的泼尼松,能够有效缓解患者疼痛程度,提高诊疗的舒适度及满意度。Objective：To evaluate the effect of preventive medication on pain after one-time root canal therapy in patients with irreversible pulpitis and pulp necrosis.Methods：Ninety cases who met the experiment were selected（46 cases were diagnosed as irreversible pulpitis and 44 cases were diagnosed as pulp necrosis）.The patients were randomly divided into the experimental group（prednisone group）and the control group（placebo group）,45 cases in each group.All patients were treated with one time root canal 30 minutes before operation,and the pain degree were evaluated by visual analogue scale（VAS）at 6 h,12 h,24 h,48 hafter operation respectively.The pain accurrent rate and treatment comfort were evaluated at 24 h after operation.Results：There was a significant difference in VAS pain scores between the two groups at the observation time point（P〈0.05）.The pain occurrence rate in experimental group were lower than that in control group（P〈0.05）,and the scores of general comfort questionnaire（GCQ）in the experimental group was higher than that in control group,the difference was statistically significant（P〈0.05）.Conclusion：Preemptive analgesia with prednisone can effectively relieve the patient′s pain and improve the comfort and satisfaction of the patient′s diagnosis and treatment.
