老年三阴性乳腺癌患者血清miR-21、-200a、-200b水平及临床意义

摘　　要：目的探讨血清miR-21、-200a、-200b水平在老年三阴性乳腺癌患者中的临床意义。方法分别将80例经临床病理学确诊的老年三阴性乳腺癌患者和50名健康体检者作为观察组和对照组,采用荧光实时定量PCR技术测定两组血清miR-21、-200a、-200b水平,对不同分化程度、不同临床分期和有无淋巴结转移患者血清miR-21、-200a、-200b水平进行比较。结果与对照组比较,观察组血清miR-21、-200a水平显著升高,miR-200b水平显著降低(P均<0. 05);与中高度分化、Ⅰ～Ⅱ期和无淋巴结转移的老年三阴性乳腺癌患者比较,低分化、Ⅲ～Ⅳ期和有淋巴结转移的老年三阴性乳腺癌患者血清miR-21、-200a水平显著升高,miR-200b水平显著降低(P均<0. 05)。结论老年三阴性乳腺癌患者血清miR-21、-200a水平明显升高,miR-200b水平明显降低,且与肿瘤分化程度、临床分期、淋巴结转移密切相关,均可作为三阴性乳腺癌严重程度及预后的判断指标。Objective To investigate the clinical significance of serum levels of miR-21,miR-200a and miR-200b in elderly patients with triple-negative breast cancer.Methods 80 elderly patients with triple-negative breast cancer were collected as the observation group,and 50 healthy people who received heath check-up were collected as the healthy control group. Serum levels of miR-21,miR-200a and miR-200b were tested by realtime PCR and compared between the two groups. Then,80 elderly patients with triple-negative breast cancer were divided into different groups according to clinical characteristics. And serum levels of miR-21,miR-200a,miR-200b were compared between different groups of different degree of differentiation,clinical stages and status of lymph node metastasis. Results In comparison with the healthy control group,the observation group had higher serum levels of miR-21,miR-200a and lower serum level of miR-200b with all statistical differences （all P〈0.05）. Compared with middle and highly differentiated,Ⅰ ～ Ⅱstage and lymph node metastasis free elderly patients with triple-negative breast cancer,the low differentiated,Ⅲ ～ Ⅳ stage and lymph node metastasized elderly patients with triple-negative breast cancer had higher serum levels of miR-21,miR-200 a and lower serum level of miR-200 b with all statistical differences （all P〈0.05）.Conclusions Elderly patients with triple-negative breast cancer has significant higher serum levels of miR-21,miR-200 a and lower serum level of miR-200 b,which are closed related to degree of differentiation,clinical stages and lymph node metastasis. All of miR-21,miR-200 a and miR-200 b could be indices of severity and prognosis for elderly patients with triple-negative breast cancer.
