血清降钙素原水平对急性脑梗死病情及预后的评估价值

摘　　要：目的探讨血清中降钙素原(PCT)水平对急性脑梗死病情判断及预后评估的价值。方法选取2014年10月—2015年10月我院收治的120例急性脑梗死病人,分析病人入院前后PCT水平与病人病情之间的关系,并对病人的预后情况进行分析。结果大面积梗死组血清PCT水平和美国国立卫生研究院卒中量表(NIHSS)评分均高于中等面积梗死组以及小面积梗死组(P <0.05);中等面积梗死组血清PCT水平和NIHSS评分均高于小面积梗死组(P <0.05);病人在入院后血清中PCT水平均有一个上升的过程,大致在第3天到达顶峰之后又逐渐下降; PCT>10μg/L病人脑梗死复发或加重、死亡、下肢深静脉血栓形成、心肌梗死的发生率高于PCT<10μg/L病人(P <0.05); PCT 5μg/L～10μg/L病人死亡率高于PCT<5μg/L病人(P <0.05)。PCT>10μg/L病人的长期无复发生存率较PCT<10μg/L病人低,差异有统计学意义(P <0.05)。结论 PCT能够及时地反馈病人的病情,并指导病人的治疗,能够用于判断病人的预后状况。Objective To investigate the value of serum procalcitonin in the diagnosis and prognosis of acute cerebral infarction pa -tients.Methods One hundred and twenty patients with acute cerebral infarction admitted from October 2014 to October 2015 wereenrolled in this study.The relationship between the level of procalcitonin （PCT） before and after admission was analyzed,and its rela -tion with the prognosis of the patients was also analyzed.Results The serum levels of PCT and NIHSS score in the large area in -farction group were significantly higher than those in the medium and small area infarction groups （P 〈0.05）.The serum PCT level inthe medium area infarction group was significantly higher than that in the small area infarction group （ P 〈0.05）.There was a rise inthe serum level of PCT after admission,and the serum level of PCT gradually decreased after the peak on the third day.The serumPCT level was higher than that in the control group （P 〈0.05）,the incidence of myocardial infarction was higher in those with PCT〉10μg/L than PCT〈10μg/L,the difference was statistically significant （P 〈0.05）.The long term recurrence free survival rate of thepatients with high PCT level was lower than that of patients with middle and low PCT levels （ P 〈0.05）,with statistically significantdifference （P 〈0.05）.Conclusion PCT might feedback the patient ′s condition,and guide the treatment of patients,which could beused to determine the prognosis of patients.
