摘 要:目的探讨核型自动扫描系统与双人盲法阅片技术在医疗放射人员外周血淋巴细胞畸变检测应用中的优劣。方法利用核型自动扫描系统及双人盲法阅片技术对400份从事医疗放射人员外周血液淋巴细胞吉姆萨染色玻片样本进行检测,对检测结果进行统计学分析。结果针对两组各400份样本平均检测用时统计分析发现,扫描组为每人(19.39±0.78)min,人工组为每人(36.77±1.01)min,差异有统计学意义(t=149.321,P<0.05);针对微核细胞数目均值统计分析发现,两组各检测淋巴细胞4×105个,扫描组为(2.70±1.71),人工组为(1.94±1.59),差异有统计学意义(t=-8.718,P<0.05),同时两组样本微核阳性检测数据呈正相关(r=0.723,P<0.05);针对染色体畸变细胞数目均值统计分析发现,两组各检测淋巴细胞8×104个,扫描组为(0.56±0.64),人工组为(0.05±0.22),差异有统计学意义(t=-12.034,P<0.05),同时两组样本染色体畸变阳性检测数据呈正相关(r=0.363,P<0.05);针对两组各400份样本阳性率统计分析发现,扫描组为13.00%,人工组为9.50%,差异无统计学意义(Mc Nemar精确检验,P>0.05)。结论核型自动扫描系统检测法拥有较快的阅片速度,且漏检率低,适合数目较大的玻片样本分析;双人盲法阅片检测技术成本较低,且操作更加灵活简便,适合小样本量样品分析。两种方法可互补使用。Objective To discuss the advantages and disadvantages of automatic scanning system tests and artificial double blind reading tests for medical radiation workers' peripheral blood lymphocyte aberrations. Methods Totally 400 samples of medical radiation workers' peripheral blood lymphocytes were tested using automatic scanning system tests and artificial double blind reading tests,and the differences on different detection methods were statistically analysed. Results Statistical analysis of the average detection time for the two groups of 400 samples found that the it was( 19. 39 ± 0. 78) min per person in the scanning group,and( 36. 77 ± 1. 01) min per person in the artificial group. The differences were statistically significant( t = 149. 321,P〈0. 05). According to the statistical analysis of the average number of micronuclei cells,the number of lymphocytes detected was 4 × 105 in each group,being( 2. 70 ± 1. 71) in the scanning group,and( 1. 94 ± 1. 59) in the artificial group. The differences were statistically significant( t =-8. 718,P〈0. 05). At the same time,there was a positive correlation between positive detection data of micronucleus in two groups of samples( r = 0. 723,P〈0. 05). According to the statistical analysis of the mean number of chromosome aberrations,8 × 104 lymphocytes were detected in each group,being( 0. 56 ± 0. 64) in the scanning group and( 0. 05 ± 0. 22) in the artificial group. The differences were statistically significant( t =-12. 034,P〈0. 05). At the same time,the positive detection data of chromosomal aberrations in both groups were positively correlated( r = 0. 363,P〈0. 05).
