一次性根管治疗后冠修复在隐裂性牙髓炎治疗中的临床应用

摘　　要：目的:通过临床观察研究一次性根管治疗后冠修复对隐裂性牙髓炎的治疗效果。方法:将我院2013年7月～2015年6月期间隐裂性牙髓炎患牙269颗分别编号随机分为两组,实验组135颗患牙经一次性根管治疗后行冠修复,对照组134颗患牙经过多次根管治疗后冠修复。对两组患牙术后1周疼痛程度、术后半年X线及术后2年治疗效果进行对比分析。结果:对比两组患牙术后1周疼痛发生率分别为8. 89%～9. 63%及8. 21%～10. 4%,差异无统计学意义(P> 0. 05)。半年X线片改变分别为2. 22%及7. 46%～8. 21%,差异具有统计学意义(P <0. 05)。2年随访治疗有效率分别为93. 3%～94. 8%及85. 1%～85. 8%,差异具有统计学意义(P <0. 05)。结论:一次性根管治疗后冠修复对隐裂性牙髓炎治疗预后效果明显,值得临床推广应用。Objective：To investigate the prognosis of one-time root canal therapy with crown restoration in cracked tooth with pulpitis, nethods：A total of 269 cracked teeth with pulpitis were randomly divided into 135 in treatment group and 134 in control group receiving either one-time root canal therapy or regular ways followed with crown restoration. These two groups were compared and analyzed from the aspects of the pain after one week, the changes of X- ray characters ＇after 6 months and the 2 years successful rate. Results：The incidence of pain in the two groups was 8.89% -9.63% and 8.21% 10. 4% after 1 week, and there was no significant difference between the two groups. The X-ray changes in half a year were ：2.22% and 7.46% -8.21%, and there was statistically significant difference between the two groups（P 〈 0. 05 ）. The suc- cess rate after 2 years of treatment group was 93.3% - 94. 8% in comparison with 85.1% - 85.8% of control group, and there was statistically significant difference between the two groups （P 〈 0. 05 ）. Conclusion：Cracked tooth with pulpitis trea- ted by one- time root canal therapy with crown restoration had better prognosis than regular treatment. So one- time root canal therapy with crown restoration is worth popularizing and applying in clinical treatment.
