单、双相抑郁障碍患者脑葡萄糖代谢的对比研究

摘　　要：【目的】通过对不同亚型抑郁障碍患者脑葡萄糖代谢的检测,探讨单、双抑郁障碍患者的脑局部代谢之间是否存在差异。【方法】对符合DSM-Ⅳ的单相抑郁障碍和双相抑郁障碍患者各12例和10例正常对照组进行静息态正电子发射计算机断层/18F-氟代脱氧葡萄糖(PET/FDG)扫描,利用参数统计图(SPM5)方法分析组间脑局部代谢的分布差异。【结果】与正常对照组相比,双相抑郁障碍组的右侧颞下回和颞中回、左侧颞中回、额中回,顶下叶左侧脑区的FDG代谢水平明显低于对照组(P<0.05);而单相抑郁障碍组的左侧额中回,右侧额下回脑区的FDG代谢水平明显低于对照组(P<0.05);双相抑郁障碍组的左侧额叶眶回、右侧额叶中央旁小叶的FDG代谢水平高于单相抑郁障碍组(P<0.05)。【结论】单、双相抑郁障碍患者存在脑局部代谢异常模式。且不同亚型的抑郁障碍患者的额叶部分区域的葡萄糖代谢存在统计学差异。[Objective ] To explore the difference of cerebral glucose metabolism between unipolar and I）ipolar depression. [ Methods] There were 12 patients wilh unipolar depression and 12 eases with bipolar depression included, according to DSM-IV. And 10 healthy per- sons were selected as control. The [18F] fuorodeoxyglueose （FDG） positon emission tomography （PET） was used to evaluated the difference in glucose metabolism at resiing state. The PET data were objectively analyzed by statistical parametric mapping （SPM）. [ Hesults] Com- pared with the normal control group, the glucose metabolism in patients with bipolar depression was significantly lower in the leli middle frontal gynJs, bilateral middle temporal g＇yrus, fight inferior temporal gynls and left irfferior parietal ]obule （P〈O.05）. For the unipolar de- pression, the ghleose metabolism was significantly lower in the left middle frontal gyrus and right inferior frontal g3.,rns （P〈O.05）. Compared with the unipolar depression, the glucose metabolism was higher in left orbital frontal gyrus and right frontal gyrus in patients wilh bipolar depression （P〈O.05）. [ Gonclusion ] There are cerebral glucose metabolism abnormalities in patients with unipolar and bipolar depression. Moreover, there is a statistical difference i,l glucose metabolism in parts of the frontal lobe between different subtypes.
