摘 要:目的探讨宫颈非典型鳞状细胞中人乳头瘤病毒(HPV)感染基因型分布。方法选取2017年7月至2018年7月间江苏省无锡市妇幼保健院收检的50名女性体检者,均行宫颈细胞学检查提示为非典型鳞状细胞。提取14种人乳头瘤病毒感染基因,采用基因扩增结合基因芯片技术进行基因分型检测,分析其检测结果。结果受检者中检出人乳头瘤病毒感染11例(22. 0%),其中高危型(16、18、31、33、35、39、45、51、52、56、58、59、66、68) 10例(90. 9%),低危型(16、18、45、52、58)1例(9. 1%);高危型检出率高于低危型,差异有统计学意义(P <0. 05)。11例中单型感染7例(63. 6%),多型感染4例(36. 4%),单型感染检出率高于多型感染,差异有统计学意义(P <0. 05)。HPV感染基因型分布中,前4位是16(26. 0%)、18 (14. 0%)、52 (10. 0%)和58 (8. 0%)。高危型组与低危型组的16、18、52和58亚型构成比相当,差异均无统计学意义(P> 0. 05)。结论宫颈非典型鳞状细胞中人乳头瘤病毒感染率较高,运用基因扩增结合基因芯片技术对其进行检测分析,可促进宫颈非典型鳞状细胞的进一步分流管理。Objective To study the distribution of human papilloma virus( HPV) genotypes in cervical atypical squamous cells. Methods Fifty women who underwent cervical cytological examination at Wuxi Maternal and Child Health Hospital from July 2017 to July 2018 were selected. The examination results suggested all the women had cervical atypical squamous cells. Fourteen HPV infection genes were extracted from atypical squamous cells. Gene amplification and gene chip technology were used for genotyping detection. Results Among the patients,HPV infection were detected in 11 patients( 22. 0%),including10 with high-risk type( 90. 9%,16,18,31,33,35,39,45,51,52,56,58,59,66 and 68) and 1 with low-risk type( 9. 1%,16,18,45,52 and 58). The detection rate for high-risk type was higher than that for low-risk type( P〈0. 05). Among the above 11 patients monotypic infection was found in 7 patients( 63. 6%) and polytypic infection was found in 4 patients( 36. 4%) with the former significantly higher than the latter( P〈0. 05). In the distribution of HPV genotypes,16( 26. 0%),18( 14. 0%),52( 10. 0%) and 58( 8. 0%) were the top four. The composition ratio of 16,18,52 and 58 subtypes in high-risk group was similar to that in low-risk group( P〉0. 05). Conclusion HPV infection rate is high in cervix atypical squamous cells. Detection and analysis of HPV using gene amplification combined with gene chip technology can promote further shunt management of them.
