中医扶正培本联合化疗对晚期非小细胞肺癌患者免疫功能及循环肿瘤细胞水平的影响

摘　　要：目的探讨中医扶正培本联合化疗治疗对晚期非小细胞肺癌患者免疫功能及循环肿瘤细胞水平的影响。方法选取2016年5月至2018年1月间南通市肿瘤医院收治的60例晚期非小细胞肺癌患者,采用随机数表法分为治疗组和对照组,每组30例。对照组患者采用康莱特联合顺铂+多西他赛(DP方案)化疗,治疗组患者在对照组基础上加中医扶正培本治疗,比较两组患者的治疗效果、治疗前后免疫细胞功能指标变化、治疗后的循环肿瘤细胞水平及生活质量评分情况。结果治疗后,两组患者的CD3^+、CD4^+、CD4^+/CD8^+和NK水平均升高,且治疗组患者上述指标升高更显著,差异均有统计学意义(均P <0. 05)。治疗组患者各级循环肿瘤细胞阳性数量均较对照组低,差异均有统计学意义(均P <0. 05)。治疗组患者总健康状况评分较对照组高,疲倦和恶心呕吐评分均较低,差异均有统计学意义(均P <0. 05)。治疗组患者的肿瘤缓解率为56. 7%,明显高于对照组的23. 3%,差异有统计学意义(P <0. 05)。结论中医扶正培本联合化疗治疗非小细胞肺癌患者,可明显增强机体免疫功能,抑制肿瘤细胞转移,提高生活质量,效果显著,值得临床推广使用。Objective To explore the effect of Chinese medicine Fuzheng Peiben combined with chemotherapy on immune function and circulating tumor cell level in patients with non-small cell lung cancer. Methods A total of 60 patients with advanced non-small cell lung cancer admitted to Nantong Tumor Hospital from May 2016 to January 2018 were selected and randomly divided into a treatment group and a control group,with 30 patients in each group. The patients in the treatment group were treated with kanglaite combined with cisplatin and docetaxel（ DP regimen） plus traditional Chinese medicine Fuzheng Peiben. The patients in the control group were only given kanglaite combined with DP regimen. Treatment efficacy,changes in immune cell function measurement indexes before and after treatment,circulating tumor cell levels and quality of life scores were compared between the two groups. Results After the treatment,CD3^＋,CD4^＋and CD4^＋/CD8^＋ and NK levels increased and the increase in the above indexes was more significantly in the treatment group than in the control group（ all P〈0. 05）. The amount of positive circulating tumor cell was lower in the treatment group than in the control group（ all P〈0. 05）. The overall health scores were higher in the treatment group than in the control group but nausea and vomiting scores were lower in the treatment group than in the control group（ all P〈0. 05）. The tumor remission rate was 56. 7%for the treatment group,which was significantly higher than 23. 3% of the control group（ P〈0. 05）.Conclusion Traditional Chinese medicine Fuzheng Peiben combined with chemotherapy can significantly enhance immune function,inhibit the metastasis of tumor cells and improve quality of life in patients with advanced non-small cell lung cancer,and the effect is significant,which is worthy of clinical promotion.
