探讨脑梗死患者血清同型半胱氨酸水平与脑梗死发病与预后的关系

摘　　要：目的探讨脑梗死患者血清同型半胱氨酸(Hcy)水平与脑梗死发病及预后的关系。方法选择2015年6月～2017年10月72例脑梗死患者,72例患者按脑梗死分期分为急性期(发病<72 h)与恢复期(发病>14 d);选择同期45例健康者为对照组。行血清Hcy水平检测,分析血清Hcy与梗死灶大小、病情严重程度、脑梗死分期及预后的关系。结果脑梗死患者的血清Hcy水平明显高于对照组(P<0.05);不同大小梗死灶血清Hcy水平差异无统计学意义;脑梗死病情中度患者血清Hcy水平明显高于轻度(P<0.05),重度明显高于中度(P<0.05);脑梗死急性期血清Hcy水平明显高于恢复期(P<0.05);脑梗死预后良好患者血清Hcy水平明显低于预后不良者(P<0.05)。结论血清Hcy可作为脑梗死的辅助诊断及病情监控、预后评估指标。Objective To investigate the relationship between serum homocysteine （Hcy） and the incidence and prognosis of cerebral infarction in patients with cerebral infarction. Methods From June 2015 to October 2017, 72 patients with cerebral infarction were enrolled. 72 patients were divided into acute phase （onset 〈 72 h） and recover）, period （onset2〉 14 d） according to the stage of cerebral infarction. 45 healthy controls were selected as the control group. Serum Hcy levels were measured to analyze the relationship between serum Hcy and infaret size, severity of disease, cerebral infarction stage and prognosis. Results Serum Hcy level in patients with cerebral infarction was significantly higher than that in control group （P〈0.05）. There was no significant difference in serum Hcy levels between different infarct sizes. Serum Hcy levels in patients with moderate cerebral infarction were significantly higher than those in mild （P〈0.05）, and the severity was significantly higher than that of the moderate （P〈0.05）. The serum Hcy level in the acute stage of cerebral infarction was significantly higher than that in the recovery stage （P〈0.05）. The serum Hcy level in patients with good prognosis of cerebral infarction was significantly lower than that in the poor prognosis （P〈0.05）. Conclusion Serum Hcy can be used as an auxiliary diagnosis of cerebral infarction and disease monitoring, prognostic evaluation indicators.
