血脂及纤维蛋白原与急性缺血性脑卒中伴出血转化的关系研究

摘　　要：目的探讨缺血性脑卒中伴出血转化的患者与血脂及纤维蛋白原的相关性,以便指导临床病情评估及治疗。方法选取2015年4月～2016年10月就诊本院的101例急性缺血性脑卒中患者为研究对象,按是否发生出血转化分为出血转化组(HT组,26例)和非出血转化组(非HT组,75例)。所有研究对象均于入院第二日采空腹血,并测定总胆固醇(TG)、甘油三酯(CHO)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL-C)及纤维蛋白原(FIB)、空腹血糖(Glu)水平。利用SPSS 17.0软件进行数据分析统计。结果 HT组TG、CHO、LDL-C水平明显低于非HT组,差异有统计学意义(P<0.05);HT组FIB水平明显低于非HT组,差异有统计学意义(P<0.05)。结论急性缺血性脑卒中患者低水平血脂及FIB与出血转化的发生有关。因此,对于出血转化的高危人群,需慎用降脂及降纤治疗。Objective To explore the correlations between acute ischemic stroke with cerebral hemorrhage transformation （HT） and lipids and fibrinogen, it＇ s important for evaluating prognosis and guiding therapy. Methods Atotal of 101 patients with actue ischemic stroke （IS） in our hospital from April 2015 to October 2016 were examined. The patients with cerebral HT were divided into the HT group, the others were into the control group. The levels of lipids, fibrinogen and glucose of all the patients were tested. Data were analyzed using the SPSS 17.0 statistical software. Results The level of TG, CHO, LDL-C in the HT group were significantly lower than the control group （P 〈0.05）. The level of fibrinogen wasobviously lower than the control group （P 〈0.05）. Conclusion The low levels of lipids and fibrinogen in patinets with IS are correlated with cerebral HT. Therefore, physicians should be careful of lipid-lowering and fibrinogen treatment for the patinets at high risk of cerebral HT.
