开颅夹闭术与血管内介入治疗颅内动脉瘤患者血清MBP、NSE、S100B水平的影响

摘　　要：目的探讨开颅夹闭术与介入治疗对颅内动脉瘤(IA)患者血清髓鞘碱性蛋白(MBP)、神经元特异性烯醇化酶(NSE)及S100B蛋白表达的影响。方法将70例IA患者根据手术方式分为开放组和介入组,各35例。开放组接受开颅夹闭术治疗,介入组进行血管介入栓塞治疗,对比两组手术时间、住院时间、费用以及预后情况,观察两组患者治疗前后MBP、NSE及S100B水平变化。结果介入组患者手术时间、住院天数均明显小于开放组,而住院费用则显著高于开放组(P<0.05)。介入组治疗后预后良好率为88.57%,显著高于开放组68.57%(P<0.05)。两组患者治疗后血清MBP、NSE及S100B水平均明显降低,且介入组患者各血清指标均低于开放组(P<0.05)。介入组术后并发症发生率明显低于开放组,差异有统计学意义(P<0.05)。结论血管介入栓塞可缩短IA患者手术和住院时间,改善预后状况,减轻神经功能损伤,具有较高安全性。Objective To investigate the influence of craniotomy and endovascular embolization on serum myelin basic protein （MBP）, neuronspecific enolase （NSE） and S100B in patients with intracranial aneurysm（IA）. MethodSeventy patients with IA were divided into open group and endovascular group according surgical method, with 35 cases in each group. Patients in open group were received IA clipping and patients in endovascular group were treated with endovascular embolization. The time of operation, hospital stay, expenses and prognosis condition were compared in two groups. The changes of serum MBP, NSE and S100B in two groups were observed before and after treatment. Results The operation time and hospital stay of endovascular group were significantly shorter than open group, while the expenses was significantly higher than open group （P〈0.05）. The rate of favourable prognosis in endovascular group was 88.57%, which was significantly higher than 68.57% of open group （P〈0.05）. The levels of MBP, NSE and S100B were significantly reduced in two groups after treatment, and those indexes in endovascular group were significantly lower than open group （P〈0.05）. The rate of postoperative complications in endovascular group were significantly lower than open group （P〈0.05）. ConclusionEndovascular embolization can significantly short the time of operation and hospital stay, improve prognosis, reduce nerve function injury and also has high security.
