摘  要:评估子宫上皮细胞稳定性FH检测(简称FH检测)在宫颈癌、癌前病变及宫颈病变(包括宫颈癌及癌前病变)筛查中的临床价值。使用FH检测、宫颈液基细胞学检查(TCT)及人乳头状瘤病毒(HPV)检测3种检查方法进行宫颈癌、癌前病变及宫颈病变的筛查。对3种检查任一项或一项以上呈阳性的患者,进行阴道镜下宫颈活检术及病理检查。以病理结果为金标准,比较FH、TCT、HPV检测宫颈癌、癌前病变及宫颈病变的敏感度、特异度。TCT和FH检测宫颈癌、宫颈癌前病变及宫颈病变的敏感度差异无统计学意义(P值分别为0. 303,0. 492,0. 672);HPV和FH检测宫颈癌的敏感度差异无统计学意义(P=0. 649),检测宫颈癌前病变及宫颈病变方面,HPV的敏感度显著大于FH检测(P均<0. 01); FH检测宫颈癌、宫颈癌前病变、宫颈病变的特异度大于TCT、HPV(P均<0. 01)。FH检测具有较好的检测敏感度、特异性,操作简便,可快速得到检测结果,成本低,可用于经济不甚发达地区及基层医院、诊所宫颈癌及宫颈癌前病变的筛查。Cervical cancer is a common malignant tumor in woman. Screening of cervical cancer and precancerous lesions can effectively reduce the morbidity and mortality of cervical cancer. We qualitatively detected cells ferrous free Protoporphyrin(FH) in cervical exfoliated cells to screen cervical cancer and precancerous lesions. We evaluated the clinical value of uterine epithelial cells stability FH test in the screening of cervical cancer,precancerous lesions and cervical lesions including cervical cancer and precancerous lesions. FH test,Thinpep cytologic test(TCT) and human papillomavirus(HPV) test were used to screen the cervical cancer,precancerous lesions and cervical lesions. The cervical biopsy and pathological examination under colposcopy were performed on the patients with one or more than one positive test. Based on the golden standard of cervical tissues pathology examination,we compared the sensitivity and specificity of FH,TCT and HPV test in cervical cancer and precancerous lesions and cervical lesions.There was no significant difference in sensitivity of TCT and FH for detecting cervical cancer,precancerous lesions and cervical lesions(P value was 0. 303,0. 492,0. 672,respectively). There was no significant difference in the sensitivity of HPV and FH for detecting cervical cancer(P = 0. 649). In detecting cervical precancerous lesions and cervical lesions,the sensitivity of HPV was significantly greater than that of FH(P〈0. 01). The specificity of FH for the detection of cervical cancer,precancerous lesions and cervical lesions was greater than that of TCT and HPV(P〈0. 01). FH test has good sensitivity and specificity,simple operation,quick detection results,and low economic cost. FH test can be used in backward areas and primary hospitals to expand the scale of the cervical cancer and precancerous lesions screening,and reduce the morbidity and mortality of cervical cancer.
