康艾结合灌肠治疗食管癌术后并发肾衰竭的疗效观察

摘　　要：目的探讨康艾结合灌肠治疗食管癌术后并发肾衰竭对患者血清相关因子、肾功能、临床疗效及生活质量的影响,为本病的诊疗提供新的临床思维。方法将160例诊断为食管癌术后并发肾衰竭的患者采用随机数字表法,平均分为对照组和研究组,每组80例。对照组采用常规饮食控制及西医对症治疗,研究组在对照组的基础上给予康艾联合益气补肾方及中药保留灌肠方治疗。30 d为1个疗程,连续治疗2个疗程。治疗结束后观察并对比两组患者的血清相关因子、肾功能、临床疗效及生活质量的变化情况。结果治疗前后研究组血清CEA、SSCA水平和临床疗效、生活质量改善情况及中医证候积分明显优于对照组,差异有统计学意义(P <0. 05);治疗前后研究组BUN、SCr及CCr水平与对照组比较未见明显变化,差异无统计学意义(P> 0. 05)。结论康艾结合灌肠治疗食管癌术后并发肾衰竭的血清CEA、SSCA水平、临床疗效及生活质量改善情况显著,值得临床进一步推广应用,但对于肾功能的相关指标未见明显改善,需进一步研究探讨,为疾病的诊治提供新的临床思维。Objective To investigate the effects of Kang-ai combined enema in treatment of esophageal cancer complicated with renal failure after surgery on serum related factors, renal function and clinical efficacy and life quality of patients in order to provide new thinking for the diagnosis and treatment of the disease. Methods A total of 160 patients who were diagnosed as renal failure after esophageal cancer surgery were enrolled in thestudy. According to random number table, these patients were divided into control group （n = 80） and research group （n = 80）. The patients in control group were given routine diet control and western medicines symptonlatic treatment, however,thepatients in research group, on the basis of control group ,were treated by Kang-ai combined with Yiqibushen prescription and TCM retention enema,with a treatment course being 30 day-s for continuous course treatment of 2 treatment courses for both groups. After treatment the changes of serum correlation factor, renal function, clinical efficacy and life quality- of patients were observed and compared between the two groups. Results The serum levels of CEA, SSCA, and clinical efficacy, life quality of patients and TCM symptom scores in research group were superior to those in control group （P 〈 0.05 ）. However there were no significant differences in the levels of BUN, SCr and CCr before and after treatment between the two groups （P 〉 0.05 ）. Conclusion Kang-ai combined enema in treatment of renal failure after esophagectomy can improve serum levels of CEA and SSCA of patients,improve clinical curative effects and quality of life significantly, therefore, it is worthy of further clinical application and promotion.
