介入栓塞术治疗颈内动脉-后交通段动脉瘤的临床疗效分析

摘　　要：目的分析介入栓塞术治疗颈内动脉-后交通段动脉瘤的复发情况及与形态学的关系。方法选取吉林大学第一医院神经外科自2013年9月至2017年6月行介入栓塞治疗的起源于后交通动脉的动脉瘤患者57例。根据头数字减影血管造影将患者分为:a型、b型及c型。采用单纯栓塞及支架辅助栓塞治疗,依据Raymand分级评估栓塞疗效,及术后6个月～1年的复发率。结果a型9例;b型33例;c型15例。57例随访患者中,复发11例,术后总体复发率为19.3%。其中b型动脉瘤中,复发患者为7例,复发率为21.2%;c型动脉瘤中,复发患者为4例,复发率为26.7%。结论颈内动脉-后交通段动脉瘤栓塞术后复发集中于b型及c型动脉瘤。Objective To analyze the recurrence of internal carotid artery-posterior communicating segment aneurysms treated by interventional embolization and its relationship with morphology. Methods Fifty-seven patients with aneurysms originating from posterior communicating artery treated by interventional embolization from September 2013 to June 2017 in Department of Neurosurgery, The First Hospital of Jilin University, were chosen in our study. These patients were divided into type a, b and c according to DSA, and received simple embolization and stent assisted embolization. The embolization efficacy was evaluated according to Raymand classification, and the effect was reviewed after 6 months to 1 year. Results There were 9 cases of type a, 33 cases of type b, 15 cases of type c. Of the 57 patients who followed up, 11 patients recurred, and the overall recurrence rate was 19.3%. Of the type b aneurysms, 7 patients recurred and the recurrence rate was 21.2%; and of type c, 4 patients recurred, and the recurrence rate was 26.7%. Conclusion Recurrence of internal carotid artery-posterior communicating artery after embolization focused on type b and type c aneurysms.
