二维彩色多普勒血流显像在颈动脉体瘤诊断中的应用

摘　　要：目的 :探讨二维彩色多普勒血流显像 (2 D- CDFI)在颈动脉体瘤诊断中的应用价值。方法 :对 10例颈动脉体瘤患者进行了 2 D- CDFI检查 ,并经血管造影 ,其中 9例经手术病理证实。结果 :颈动脉体瘤 2 D- CDFI表现为 :1、颈动脉分叉处见实质性低回声肿块 ,边界清晰、边缘规则或呈分叶状 ;2、肿瘤较小时 ,多位于颈总动脉分叉处 ,使颈内、外动脉间距增大 ;肿瘤较大时 ,常围绕颈总动脉、颈内、外动脉生长 ,并将这些血管包裹 ;3、肿瘤内见较丰富的彩色血流信号 ,以动脉为主 ,其中有 8例为颈外动脉的分支直接进入肿瘤内 ;4、能清晰地显示肿瘤与颈动脉的关系 ,超声诊断符合率为 10 0 %。结论 :2 D- CDFI具有分辨率高、无创、安全、特异性强等特点 。Objective:To explore the value of two dimensional color Doppler flow imaging(2D-CDFI) for the diagnosis of the carotid body tumor.Methods:Ten patients with carotid body tumors were examined by 2D-CDFI and angiography,and 9cases of the ten were proved by surgical operation.Results:The carotid body tumors in 2D-CDFI were shown as:follows:1?Low echoic substantial mass were found in the carotid branch with clearly boundary and clearly or partly outline. 2?Most of them were located at the branch of common carotid artery when they were in small size,so the interval between external and internal carotid arteries was enlarged. They always grew around CCA.ICA.ECA when they were big. 3?More color flow signals were detected in the masses. Most of them were arteries, Among them,8 cases were detected and shown that branches of externat carotid arterials extended into the masses directly.4?They could be shown clearly the relationship between the mass and the carotid. The correct diagnostic rate of ultrasound were 100%.Conclusions:2D-CDFI is a valuable and practical method for diagnosis of carotid body tumors.
