摘　　要：目的 :总结保留十二指肠升部的胰头十二指肠切除术。方法 :回顾分析我院近年 2 12例胰头十二指肠切除术采用保留十二指肠升部的 2 1例 ,描述手术要点 ,总结其优点。结果 :无手术死亡 ,术后 2例胰漏、1例胆漏、1例低热 ,皆保守治愈。生存超过 5年者有 3例 ,有超过 4年、3年的。结论 :本术式既可减少由广泛解剖所致的损伤 ,缩短手术时间 ,又不影响淋巴结的清扫。对壶腹周围癌尚未扩散者都可考虑 ,对年老体弱。Objective:To discuss the preservation of the ascending portion of the duodenum in pancreatoduodenectomy.Methods:By retrospectively analyzing 21 cases of ascending portion preserving operation in 212 cases of pancreatoduodenectomy in our hospital in recent years,we described the key points of this operation and its advantages.Results:No operation related death. Complications included 2 pancreatic leakages,1 bile leakage and 1 low fever,who were treated conservatively. 3 patients have survived for 5 years. Some survived over 3～4 years.Conclusion:This operation avoids the harmful extensive dissection and shorten the operation time with lymph node dissection unaffected. It can be used in paraampullary cancer without metastasis and especially suitable for the fat aged patients and for those with vital organ dysfunction.
