摘　　要：应用所设计的改良咽腭肌瓣手术——隧道咽腭肌瓣咽成形术治疗腭裂、腭咽闭合不全患者46例。术后半年至一年随访结果提示,腭咽闭合优良率为83.3%～88.2%。本手术可为腭裂、腭咽闭合不全患者形成功能性咽部肌肉环,并同时使鼻咽腔缩小,咽后壁增高,均有利于语音机能的恢复。A description of the modified pharyngoplasty using tunneled palatopharyngeal flap in 46 patients with cleft palate and velopharyngeal incompotence is given and the follow-up results of 32 patients between 6 months and 1 year after operation are presented. The rate of velopharyngeal competence and mild incompetence (velopharyngeal gap < 4mm) is 88.2% comfirmed by nasopharyngoscopic examination, and 83.3% with lateral X-ray radiograph. The results show that this modified pharyngoplasty accords with the velopharyngeal anatomy and physiology. We believe this operation is effective in reconstructing a new palatopharyngeal muscle loop, reducing the dimensions of nasopharynx, and augmenting the posterior pharyngeal wall. The importance that the flap is placed near the site of velopharyngeal closure is emphasized. The design, advantages and disadvantages are discussed in detail.
