摘  要:目的 探讨CD15s抗原在乳腺癌组织中的表达及分布特点以及与肿瘤发生、发展和转移的关系。方法 应用催化信号放大系统免疫组化技术 ,对乳腺癌及正常乳腺组织进行CD15s抗原检测 ,采用胶体金免疫电镜技术观察超微结构组织中CD15s抗原的分布特征。结果  94例乳腺癌中 75例CD15s表达呈阳性 ,表达阳性率为 79.8% ,10例正常乳腺组织腺管上皮细胞CD15s表达呈弱阳性。组织学分级为Ⅱ~Ⅲ级的乳腺癌中CD15s表达阳性率为 87.3 % ,显著高于Ⅰ级的表达阳性率 (69.2 % ,P<0 .0 5 ) ;在伴有淋巴结转移的乳腺癌组织中 ,CD15s表达阳性率为 90 .2 % ,显著高于无淋巴结转移组的CD15s表达阳性率 (67.4% ,P<0 .0 5 )。免疫电镜显示 ,CD15s抗原主要分布于乳腺癌细胞质的界膜、内质网、高尔基体及近细胞核膜处和正常乳腺组织腺管上皮细胞质的界膜处。结论 CD15s对判断乳腺癌恶性程度和预测淋巴结转移状况是一个具有实用价值的标志物 ,并为探讨乳腺癌的发生、发展机理提供了一个新途径。Objective To investigate the expression and distribution of CD 15s antigen in breast cancer and its relationship with carcinogenesis, progression and metastatic proclivity. [WT5”HZ]Methods [WT5”BZ]Catalyzed signal amplification(CSA) immunohistochemical technique was used to detect the expression of CD 15s antigen in breast cancer and in adjacent normal mucosa. Immunoelectromicroscopic ultrastructural localization of CD 15s antigen labelled by colloidal gold was also observed.[WT5”HZ]Results [WT5”BZ]The positive rate of CD 15s antigen expression in primary breast cancer was 79.8%(75/94). In adjacent normal mucosa (n=10) CD 15s antigen showed weaker staining. The positive rate of CD 15s antigen expression in grade Ⅱ-Ⅲ (87.3%) was notably higher than that in grade Ⅰ (69.2%, P<0.05). In patients with lymph node metastasis, the positive rate of CD 15s antigen expression was 90.2%, which was significantly higher than 67.4% in nodes with no metastasis (P<0.05). CD 15s antigen immunoreactivity was mainly localized in the border membrane of cytoplasm, endoplasmic reticulum, golgi complex and surrounding nuclear membrane in tumor tissue, and in the border membrane of cytoplasm in adjacent normal tissue. [WT5”HZ]Conclusion [WT5”BZ] CD 15s antigen is a practical parameter for evaluating the degree of malignancy and lymphatic metastatic proclivity of breast cancer. It can provide a new pathway to investigate the carcinogenesis and progression of breast cancer. [WT5”HZ]
