小切口加三维内固定术治疗颧骨骨折临床评价

摘　　要：目的:评价小切口加三维内固定术治疗颧骨骨折的临床效果。方法:38例颧骨骨折患者经术前X线片、CT或者三维CT重建成像确诊后,设计眶外缘、口腔前庭沟切口,骨折复位后分别行颧额缝、颧上颌缝、眶上缘三维方向的钛板坚固内固定。结果:所有患者术创均一期愈合。其中有3例患者颧骨突度略显不足;有4例患者术后张口度小于3.7 cm,但经过一段时间的训练后,均恢复正常;3例患者术后仍遗留有患侧眶下区麻木,但范围明显缩小;4例患者术前有复视,术后复视均消失。结论:本手术方法能最大限度的恢复患者外形,减少了手术创伤,而且达到了三维方向坚固内固定的目的,值得临床推广应用。Objective:To evaluate the effect of minimal incision and 3-dimensional internal fixation on trentment of Zygomatic fracture.Methods:Before surgery,38 cases with zygomatic fracture were diagnosed with X-ray and CT or 3-dimensional CT reconstruction.The combined incisions through the exterior eye socket incision and the transbuccal incision were applied for surgical approaches.All fracture lines were exposed during the operation and the dislocated zygoma was fixed with mini-titanium plates in 3-dimension,including zygo-frontal suture,zygo-maxillary suture and inferior orbital rim.Results:The incisions of 38cases closed up in Stage Ⅰ.In 35 of 38 patients,the facial complexions resumed after surgery and 3 cases had lightly sunk cheekface.4 cases with less than 3.7 cm in opening mouth extent after surgery resumed by training.3 cases still had light infra orbit numbness and 4 cases with diplopia were recovered after surgery.Conclusion:This method can reconstruct face shape on high level,reduce surgical trauma and achieve 3-dimensional rigid internal fixation,which is worthy for wider application.
