老年人溃疡性结肠炎的临床和内镜分析

摘　　要：目的:回顾性分析老年人(年龄≥60岁)溃疡性结肠炎的临床和内镜特点。方法:选择2000年12月-2009年12月在我院确诊为溃疡性结肠炎的住院患者252例,老年组42例,非老年组210例,比较两组间在临床症状、内镜表现和病理特点方面是否有差异。结果:老年组42例,男性29例,女性13例,年龄(65.29±4.86)岁;非老年组210例,男性122例,女性88例,年龄(38.50±12.41)岁。老年组患者中腹泻、腹痛和粘液脓血便等临床表现的发生率与非老年组无统计学意义(P>0.05)。老年组中-重度病情患者占73.3%,较非老年组多(P<0.05)。内镜下老年组病变范围以直乙状结肠为主,较非老年组多(P<0.05)。组织病理学老年组异型增生的发生率高于非老年组(P<0.05)。结论:老年人溃疡性结肠炎的临床症状与非老年组无明显差异,但病情较非老年组重,异型增生的发生率高于非老年组,应予以重视。Objective:To retrospective analyze the clinical and colonoscopic features of ulcerative colitis in patients older than 60 years of age.Methods:Between Dec.2000 to Dec.2009,a total of 252 in-patients with ulcerative colitis were divided into the elderly group of 42 cases and the younger group 0f 210 cases.clinical and colonoscopic features were compared between groups.Results:42 patients were classified as the elderly group including male 29cases,female 33 cases,mean age(65.29±4.86)years old.The other 210 patients were classified as younger group including male 122 cases,female 88 cases,mean age(38.50±12.41)years old.There was no significant difference in the incidence of clinical manifestations such as diarrhea,celialgia and stool with mucus and blood(P >0.05) between the two groups.The disease of the elderly patients was more severe than that of the younger patients(P<0.05).The lesions of the elderly patients were more located at the rectum-sigmoid colon,which occured more than in the younger patients(P<0.05).The occurrence of dysplasia in the elderly patients was higher than that in the younger patients(P<0.05).Conclusion:There was no significant difference in the incidence of clinical manifestations between the elderly and younger group.But the degree of severity of the elderly patients with ulcerative colitis is comparatively severe,and the occurrence of dysplasia is more frequent.
