乳管镜在乳腺导管内乳头状瘤诊治中的作用

摘　　要：目的:探讨乳管镜在导管内乳头状瘤诊治中的作用。方法:45例导管内乳头状瘤患者行乳管镜检查及手术治疗。结果:45例患者共60个乳管行乳管镜检查,诊断单发导管内乳头状瘤39例,多发导管内乳头状瘤6例。44例患者行溢液导管区段切除术,术后病理:导管内乳头状瘤,1例伴重度不典型增生患者行乳腺切除术。溢血乳管中导管内乳头状瘤发病率为100%。非血性溢液乳管导管内乳头状瘤发病率为25%。结论:乳管镜在导管内乳头状瘤的诊断和治疗中起决定和指导作用。对同侧非血性多发乳管溢液的患者,建议行术中乳管镜检查,精确定位病变乳管。Objective:To explore application of ductoscopy in the treatment of intraductal papilloma.Methods:45 patients with intraductal papilloma undertaken ductoscopy and operation.Results:Of 45 patients with ductoscopy,39 had single intraductal papilloma,6 had multiple Intraductal papilloma.44 patients with pathologically confirmed intraductal papilloma had regional resection.1 with severe atypical hyperplasia had mastectomy.All the ducts with bloody discharge had intraductal papilloma.25% of ducts with non-bloody discharge had intraductal papilloma.Conclusion:Ductoscopy is an important option in the treatment of intraductal papilloma.Intraoperational ductoscopy is recommended for the patients with ipislateral multiple non-bloody discharge.
