CT定位射频温控热凝术治疗三叉神经痛的疗效评价

摘　　要：目的对CT定位下进行射频温控热凝术治疗三叉神经痛 5年回顾、分析及总结。　方法随防 4 0 3例治疗后三叉神经痛患者 ,包括疼痛缓解率、复发率、并发症及面部麻木发生率等。　结果 4 0 3例患者均获访 ,其手术成功率为 10 0 % ,术后即刻疼痛缓解率 99.3% ;近期疼痛缓解率 97.5 % ,复发率 2 .4 8% ;远期疼痛缓解率 92 .1% ,复发率为 7.85 % ;总缓解率 94 .3% (380 4 0 3) ,总复发率 5 .7% (2 3 4 0 3) ;并发症发生率 7.4 % ,其中角膜麻痹发生率 2 .2 % ;面部麻木发生率为 98.7%。　结论CT定位使卵园孔穿刺技术不单纯依赖经验 ,能极大提高治疗成功率 ,从而提高疼痛缓解率 。Objective To evaluate and expound the application of CT guide positioning in rdiofregueney thermocoagulation which we have used to treat trigenimal neuralgia for five years. Methods Here 403 postoperative cases were followed up. Parameters such as mainly contain the rate of relief pain and pain recurrence, incidence rate of complications and facial numbness were analysed. Results Amony 403 patients, the postoperative successful rate was 100%, immediate relief rate of pain was 99.3%, short-term relief rate in two years was 97.5% and recurrent rate was 2.48%; the long-term relief rate in after two years was 92.1% and recurrent rate was 7.85%; The total relief rate was 94.3% and recurrent rate was 5.7%. The incidence of complications was 7.4% (corneal anesthesia 2.2%) and facial numbness was 98.2%. Conclusion Under CT-guide positioning, the puncture technique through the foramen ovale needs no only depend on experience. CT can greatly increase the successful rate and decrease the incidence of postoperative complications.
