摘 要:目的 :探讨三叉神经根的解剖学结构 ,为三叉神经痛 (TN)的治疗提供形态学的依据。方法 :从尸体 (10具 )上取三叉神经根标本 ,对三叉神经感觉根 (TNSR)进行石蜡包埋及连续切片 ,并系统观察 ;对 2例TN患者行TNSR表层神经纤维束切断术及随访。结果 :解剖及组织学上显示了TNSR的中央部分有一根巨大的神经纤维束 ,而在表层环绕着细小的神经纤维束。术中观察与组织学一致 ,2例TN患者术后 ,随访 >1 5年 ,未复发。结论 :TNSR表层神经纤维束切断术治疗TN方法可行。Objective: To investigate the anatomic structure of trigeminal nerve root and provide the morphologic base for the treatment of trigeminal neuralgia(TN). Methods:Two trigeminal nerve root and the trigeminal nerve sensory root(TNSR) were obtained from 10 anatomic cadav ers and were paraffin embedded. Clinically, 2 patients with TN were operated on using the new TNSR tractotomy,in which only surface nerve fiber bundles of TNSR were cut. Results: Histologically, it was composed that many tiny nerve fiber bundles located in the surface layer and a big nerve fiber bundle in the center of the root in the TNSR. Clinically, observation on the TNSR structure during the operation was consistent with the in vitro histological findings. Two patients were followed up for 18 months and more, no recurrence. Conclusion:The technique for TN operated with TVSR tractotomy is feasible.
