紧急胃镜诊断与治疗上消化道出血428例

摘　　要：对急性上消化道出血428例进行紧急胃镜检查,360例同时胃镜局部止血,总有效率为86.9%。其中用8mg%去甲肾上腺素治疗260例,有效率达86.2%,36例用药后再出血,5%孟氏液喷洒治疗46例,有效率82.6%;而10%孟氏液喷洒治疗大出血23例,有效率达91.3%,用8mg%去甲肾上腺素+5%孟氏液治疗的大出血27例,有效率为96.3%。我们还对局部注射无水乙醇治疗大出血进行试探,4例均告成功。此外,还经内镜对造成出血病变的原因如胃石、残留缝线等进行了镜下去除。428 Cases of acute upper gastroi-ntestinal bleeding(overt bleedingand massive bleeding)diagnosed byemergency endoscopy and 360 casesof them were treated under endosco-py at the same time.Among thecases,260 of them were treated with8mg noradrenaline /100ml,with arate of efficiency of 86.2%,36 casesin vain,Efficiency of treatment for46 cases of massive bleeding with 5%monsell solution was 82.6%,efficie-ncy of management in 23 cases ofmassive bleeding sprayed with 10%monsell solution was 91.3%,efficie-ncy of therapy in 27 patients ofmassive bleeding using both 8mg%noradrenaline and 5% monsell solu-tion was 96.3%。4 cases of massivebleeding were treated by 96% alcoholwitha 100% resultant of bleedingstop.persimmon bezoars and gastricstump sutures might result in hemo-rrhage,we also removed them endo-scopically。
