放射治疗在垂体瘤治疗中的地位(附386例分析)

摘  要:本文分析我院自1963年至1986年收治垂体瘤386例治疗情况。计单纯手术117例,单纯放疗97例,手术加放疗者172例。1980年4月以前收治119例(甲组),采用常规开颅手术切除肿瘤,放疗采用钴^(60)或直线加速器照射,1980年4月以后收治267例(乙组)。采用显微外科切除肿瘤,手术加放疗患者,配合直线加速器治疗。患者随防2~23年,甲组有效率为90.7%,乙组有效率为94%,两组结果表明手术配合放疗效果较理想,复发率均比单纯放疗或单纯手术低。作者认为,为减少复发,改善远期效果,垂体瘤手术配合放射治疗仍属必要。We reviewed 386 cases, dividing into two groups, of pituitary adenoma treated from January 1963 to January 1986. Cases in group A were treated before April 1980 and mostly diagnosed by pneumoencephalography or cerebral angiography, and the tumors were removed by conventional operation. For group B cases, diagnoses were established by GT scan and endocrintic tests and the tumors were removed by microsurgery. All patients were followed for 2-23 years, efficiency rate being 90.7% in group A, 94% in group B. Both groups showed better results in patients treated by surgery combined with postoperative irradiation. The recurrence in cases treated with radiation or surgery alone is higher. We think that the optimal treatment for pituitary adenoma is surgery combined with radiothetapy, which is promising to improve the long-term result and to decrease the rate of recurrence. The progress of CT scan and microsurgery also affords advantages to the treatment of pituitary adendma. The indications for radiotherapy of pituitary adenoma, optimal dosage and complications are discussed and some suggestions about the improvement of radiotherapeutic technique are given.
