原癌基因蛋白质C-erbB-2在乳腺良恶性病变中表达的意义

摘　　要：目的 :检测原癌基因蛋白质 C- erb B- 2在乳腺良恶性病变中的表达及意义。方法 :对 1 1 1例乳腺良恶性病变的石蜡切片标本进行免疫组织化学染色。结果 :乳腺良恶性病变中乳腺导管内乳头状瘤病的 C- erb B- 2阳性率为 31 .3% ,与乳腺纤维腺病和纤维腺瘤比较差异有显著性 ( P<0 .0 1 ) ,乳腺癌 C- erb B- 2的阳性率为 74.3% ,与良性病变比较差异有显著性 ( P<0 .0 1 ) ;乳腺癌 C- erb B- 2表达与肿瘤大小和组织学分级呈正比 ( P<0 .0 5 ) ,与激素受体 ( ER、PR)水平及 5年生存期呈反比( P<0 .0 5 ) ,与淋巴结是否转移无关。结论 :C- erb B- 2表达的乳腺导管内乳头状瘤病容易癌变 ,C- erb B- 2过度表达的乳腺癌组织分化差、生存期短、预后不好。C- erb B-Objective:To study on the expression of oncoprotein C erbB 2 in breast neoplasm diseases including benign and malignant diseases and its significance.Methods:The expression of C erbB 2 was determined in paraffin embedded specimens of breast diseases including benign and malignant tumours by immunohistochemistry.Results:Positive rate of C erbB 2 expression was 31.3% in breast intraductal papillomatosis,which was higher than those in other breast benign diseases ( P <0 01). The expression rate of C erbB 2 in breast cancer was 74.3%. There was a coordinated relationship between the expression of C erbB 2 and tumor size and both histological grade in breast cancer ( P <0 05) but inverse relationship between the expression of C erbB 2 and both level of estrogen receptor (ER), progestogen receptors (PR) and the survival duration for five years ( P <0 05). There was no any relationship between the expression of C erbB 2 and the metastases of lymph nodes.Conclusion:C erbB 2 expression may be considered as a efficient factor identifying the early canceration of the intraductal papillomatosis and evaluating the prognosis of breast cancer.
