外斜V征手术探讨

摘　　要：目的 :探讨外斜V征的特征及不同术式的手术效果。方法 :5 2例外斜V征 45例行下斜肌切断 /部分切除 ,7例行水平直肌垂直移位术 ,观察手术前后的眼位、下斜肌功能和双眼视觉。结果 :下斜肌切断 /部分切除适用于下斜肌功能亢进 ～ 的外斜V征 ,水平直肌垂直移位术适用于无下斜肌功能亢进或下斜肌功能亢进 +的外斜V征。结论 :下斜肌功能亢进是外斜V征的主要原因 ,对外斜V征患者应依据是否伴有下斜肌功能亢进及亢进程度 ,行下斜肌切断 /部分切除或水平直肌垂直移位术。Objective:To investigate the clinical features and the effects of different surgical procedures for V pattern exotropia. Methods:52 cases of exotropia with V pattern were studied.The procedure of weakening inferior oblique muscle was performed on 45 cases and 7 cases were treated with the horizontal rectus vertical transposition.Eye position,the function of inferior oblique muscle and the binocular vision were analyzed. Results:The procedure of weakening inferior oblique muscle was most effective for V pattern exotropia with hyperfunction ～ of inferior oblique muscle.The horizontal rectus vertical transposition was most effective for V pattern exotropia with hyperfunction + of inferior oblique muscle or without hyperfunction. Conclusions:The hyperfunction of inferior oblique muscle is the primary factor in the etiology of V pattern exotropia.Different surgical procedures should be selected according to hyperfunction degree of inferior oblique muscle.
