摘 要:目的 :探讨化疗对晚期非小细胞肺癌患者NKC—IL - 2 -IFN -γ免疫调节功能及其血清免疫抑制作用的影响 ,为非小细胞肺癌化疗提供免疫学实验依据。方法 :采用同位素释放法和细胞病变抑制法检测 4 0例非小细胞肺癌患者外周血单个核细胞 (PBMC)的NKC—IL - 2—IFN -γ免疫调节功能 ,同时动态观察 14例晚期非小细胞肺癌患者经CAP方案化疗后NKC—IL - 2—IFN -γ免疫调节功能及血清免疫抑制作用的变化。结果 :肺癌患者NKC—IL - 2—IFN -γ免疫活性较正常对照组低。 14例接受CAP方案化疗患者在第一化疗周期中 ,化疗后患者血清对正常人PBMC产生IL - 2活性的抑制作用减弱 ,从化疗后第 2周至第 4周患者NKC、IL - 2活性均较化疗前增强 ,化疗疗程结束后 ,有效组NKC—IL - 2—IFN -γ免疫活性的增强较无效组明显 ,但都未达到正常人的水平。结论 :非小细胞肺癌患者NKC—IL - 2—IFN -γ免疫调节功能受到损害 ,患者接受CAP方案化疗后血清免疫抑制作用减弱 ,NKC—IL - 2—IFN -γ免疫调节活性在一定程度上增强 ,提示晚期非小细胞肺癌患者采用适当的化疗是有益的。Objective:To investigate chemotherapeutic influence of immuno-regulatory function and serum immuno-suppression in patients with advanced non-small-cell lung cancer (NSCLC).Methods:Samples of peripheral blood monocytes(PBMC) derived from 40 patients with advanced NSCLC and 40 normal subjects as the control were analyzed. The activity of NKC, IL-2 of PBMC were determined by isotopes release assays and IFN-γ was determined by inhibited cell lesions assays. Meanwhile 14 in 40 cases were followed up after chemotherapy of cyclophosphamide, adriamycin and cisplatin (CAP).Results:The activity of NKC, IL-2 and IFN-γ of PBMC in the patients with NSCLC was significantly lower than that in normal subjects. During the first periods of chemotherapy in 14 patients undergone chemotherapy of CAP, the effect of immuno-suppression of the sera of patients on the activity of IL-2 produced by normal human PBMC was significantly decreased after chemotherapy. While from the second week to the fourth, the activity of NKC and IL-2 was partially increased, comparing with before treatment.The activity of NKC, IL-2 and IFN-r in the responders was increased more significantly than that in the non-responders, but both failed to recover to the normal level.Conclusions:The immuno-regulatory function of NKC-IL-2-IFN-γ was reduced in patients with NSCLC, and the immuno-suppression in the serum of the patients treated with CAP was decreased. The activity of NKC-IL-2-IFN-γ, however, was to some extent increased. It indicates that it is beneficial for the patients with advanced NSCLC to be given an appropriate chemotherapy.
