摘  要:目的 探讨脑缺血早期急性肺损伤的病理改变及血浆和支气管肺泡灌洗液中内皮素 1(ET 1)含量变化。方法 用栓线法阻断大鼠右侧大脑中动脉 ,结扎对侧颈总动脉 2 4h ,观察脑与肺病理改变 ,以及血浆和支气管肺泡灌洗液中ET 1含量。结果 脑缺血组光镜下观察可见右侧脑缺血区皮质神经元缺氧变性 ;肺泡隔增宽及中性粒细胞渗出。电镜下观察脑缺血组肺泡II型上皮细胞线粒体肿胀 ,线粒体嵴断裂、消失 ,板层体数量相对减少 ,结构破坏。脑缺血组血浆ET 1(185 .87± 32 .16pg ml)明显增高 ,支气管肺泡灌洗液中ET 1含量呈增高趋势 (38.11± 10 .0pg ml)。结论 大鼠急性脑缺血早期即可出现明显的肺脏病理改变 ,血浆ETObjective To study the pathomorphological changes in lung and the changes in endothelin 1 contents in plasma and bronchoalveolar lavage fluid in rats at early stage of acute cerebral ischemia.Methods Thirty two male Sprague Dawley rats weighing 280~320 g were divided into three groups:cerebral ischemia group,sham operation group and normal control group.The right middle cerebral artery was occluded with a nylon line and the left common carotid artery was ligated for 24 hours in cerebral ischemic group.The pathomorphologic changes of lung and brain were observed with light microscope and(or) electron microscope, and ET 1 in plasma and bronchoalveolar lavage fluid(BALF) was measured by radioimmunoassay within 24 hours after the onset of acute cerebral ischemia.Results By light microscope,hypoxia of neuron in the right ischemic cortex,broadened lung alveolar septum,infiltration of neutrophils were found.Electronmicroscopic examination found that the mitochondria of the type II epithelial cells swelled, the mitochondrial cristae fragmented or disappeared, the number of osmiophilic multilamellar bodies decreased and their structures were destroyed.Plasma ET 1 levels increased in the ischemic group (185.87±32.16 pg/ml)as compared with the other two groups(P<0.05).There was no significant difference in BALF content of ET 1 between the three groups.ET 1 in BALF was correlated with that in plasma in the ischemic group.Conclusion In the early stage of cerebral ischemia in rats,there was clear pathomorphological injury in lung and increse in ET 1 in plasma.
