CD44 V6和E-cadherin在大肠癌中的表达及其意义

摘　　要：目的　研究CD4 4V6和E cadherin(E cad)在大肠癌生长转移过程中的作用 ,评价CD4 4V6和E cad可否作为预测大肠癌转移和大肠癌患者预后的生物学指标。方法　用免疫组织化学 (S P法 )对 71例大肠癌标本同时检测CD4 4V6和E cad二种指标。结果　在有淋巴结转移的病例CD4 4V6的阳性表达率为 6 6 2 % (47 71) ,明显高于无淋巴结转移的病例 (P <0 0 1)。而在高分化大肠癌E cad阳性表达率明显高于低分化大肠癌的表达率 (P <0 0 5 ) ,有淋巴结转移的病例E cad阳性表达率明显低于无淋巴结转移的病例 (P <0 0 5 )。单因素分析提示 :E cad阴性者生存率要低于E cad阳性者。COX比例风险模型多因素分析 :Dukes分期高死亡相对危险度为相应较低病人的 1 30倍、E cad阳性病人的死亡相对危险为相应阴性病人的 0 76倍。结论　CD4 4V6和E cad在肿瘤细胞的分化及肿瘤的转移过程中起着重要的作用。CD4 4V6表达与大肠癌患者的不良预后有关 ;而E cad的表达则对患者有保护趋势 ,这可为大肠癌患者的预后判断提供有效的依据。Objective To investigate the role of CD44V6 and E cad in the growth and metastasis of colorectal carcinoma,and to evaluate whether CD44V6 and E cad can be a useful marker in predicting metastatic potential of colorectal carcinoma and prognosis of the patients with colorectal carcinoma.Methods Immunohistochemical method was used to detect the expression of CD44V6 and E cad in 71 patients with colorectal carcinoma.Results The positive rate of CD44V6 was 66 2%(47/71),it was much higher in carcinoma with lymph node metastasis than that without lymphonde metastasis( P <0 01).By contraries,the positive rate of E cad in highly differentiated carcinoma was higher than that in low differentiated colorectal carcinoma.Cases with lymphnode metastasis have a lower positive rate of E cad than those without lymphnode metastasis( P <0 05). multivariate analysis revealed relative risk(RR) for mortality was lower in patients having E cad expression.Conclusion The expression of CD44V6 and E cad is correlated with progression and metastasis of colorectal carcinoma,and thus CD44V6 and E cad may be used as a predicting marker of the metastasis and prognosis of colorectal carcinoma.
