摘 要:目的 探讨铅与事件相关电位、智商及注意缺陷多动障碍 (ADHD)之间的相互关系。方法 用DSM- 诊断标准确定 ADHD60例 ,按年龄、性别 1∶ 1匹配正常对照组儿童。对所有儿童用 Conners量表进行行为问题评定 ,用原子吸收分光光度计石墨炉法及火焰法进行血及头发铅含量测定 ,应用听觉 oddball模式诱发事件相关电位 (ERP) ,采用韦克斯勒学龄儿童智力量表中国修订本 (WISC-R)法进行智能测定 ,应用 SAS统计软件对两组儿童进行分析比较。结果 t检验结果表明 ADHD患儿血铅水平明显高于对照组儿童 ,智商〔包括总智商(F IQ)、言语智商 (VIQ)、操作智商 (PIQ)〕均显著低于对照组儿童 ,事件相关电位计数正确率显著低于对照组儿童 ,P2、P3潜伏期显著延长 ,N1P2峰峰值显著低于对照组。相关分析表明血铅含量与 FIQ、VIQ、PIQ呈显著负相关 ;与 P3潜伏期、N1P2峰峰值、行为量表分呈显著正相关。P3潜伏期与血铅含量、行为量表总分呈显著正相关 ;与 FIQ、PIQ呈显著负相关。 N 1P2峰峰值与 FIQ、VIQ呈显著正相关 ,与血铅含量、行为量表分呈显著负相关。行为量表分与血铅含量 ,P2、P3潜伏期呈显著正相关 ;与 FIQ、VIQ、PIQ、N1P2峰峰值呈显著负相关。结论 ADHD儿童血铅含量较对照组儿童显著增高 。Objective To explore the relations among lead,intelligence quotient (IQ),event related potential (ERP) and attention deficit hyperactivity disorder (ADHD).Methods Sixty ADHD cases were diagnosed by using ADHD Behavior Screen with DSM Ⅳ.The control group was confirmed according to age and sex matched case control study design ,then all children′s behavior was appraised with behavior scale,WISC R examination,assaying lead in distal blood and hair with atom absorbed spectrophotometer (graphite stove and blaze method),evoked ERP with aural oddball paradigm,and analyzed data with SAS 6 12 statistics software.Results t Test results indicated lead level in distal blood with ADHD was higher than antitheses group,P<0 001;IQ (FIQ,VIQ,PIQ) was lower in ADHD,P<0 001;in ADHD group,error frequency in counting target stimulant of ERP was higher,P<0.01;P2 latency was longer,P<0.05;P3 latency was longer,P<0.01;N1P2 wave altitude was lower,P<0.02.Correlation coefficient showed blood lead with IQ,VIQ,PIQ was of negative correlation;blood lead and behavior problem scale score,P3 latency were of positive correlation;IQ and behavior problem scale score,P3 latency were of negative correlation;IQ and N1P2 wave altitude were of positive correlation;behavior problem scale score and P3 latency were of positive correlation;P3 latency and FIQ,PIQ were of negative correlation;N1P2 wave altitude and FIQ,VIQ were of positive correlation.Conclusion The blood lead in ADHD was higher,IQ was lower and P3 latency was longer than the control group.It indicated that ADHD′s brain function was immature and cognition ability was impaired.
