胃癌组织中细胞凋亡及细胞增殖的原位观察

摘　　要：　应用bcl＿2蛋白及增殖细胞核抗原(PCNA)免疫组织化学技术和原癌基因bcl＿2原位杂交技术对50例胃癌、20例胃溃疡及20例胃正常粘膜中PCNA、bcl＿2的表达进行原位观察,采用脱氧核糖核酸末端转移酶介导的dUTP末端标记(TUNEL)技术对12例胃癌、8例胃溃疡及10例胃正常粘膜中凋亡细胞进行原位观察,探讨胃癌细胞的增殖与凋亡之间的关系.发现PCNA和bcl＿2免疫组化染色阳性物质及bcl＿2原位杂交染色阳性物质在胃癌中的表达显著高于正常胃粘膜及胃溃疡中的表达;TUNEL染色阳性物质在胃癌中的表达密度显著低于胃溃疡及正常胃粘膜.在有淋巴结转移的病例中PCNA的表达显著高于无淋巴结转移的病例,而bcl＿2的表达无显著差异;Ⅰ、Ⅱ、Ⅲ级胃腺癌及不同临床病理分期(pT1、pT2、pT3)中PCNA及bcl＿2的表达无显著差异.因此,PCNA、bcl＿2的高表达及凋亡细胞的减少与胃癌的发生密切相关;bcl＿2与胃癌的发生有关,但与其发展及预后关系不大;PCNA与胃癌的发生、发展及预后均有一定关系.The expressions of bcl_2 and PCNA were detected in 50 cases of gastric carcinoma, 20 of gastric ulcer and 20 of gastric normal mucosa with immunohistochemical techniques and in situ hybridization. Cell apoptosis was observed in situ in 12 cases of gastric carcinoma, 8 of gastric ulcer and 10 of normal mucosa with TUNEL techniques. The relationship between proliferation and apoptosis of gastric carcinoma was investigated. The positive ratios of PCNA and bcl_2 expressions in gastric carcinoma were statistically higher than those in normal gastric mucosa and those in gastric ulcer. The ratio of TUNEL positive cases was statistically lower than that of normal mucosa and gastric ulcer. The positive ratio of PCNA expression in the cases with lymph node metastasis was statistically higher than that in the cases without lymph node metastasis, while the expression of bcl_2 had no statistical difference. The expression of PCNA and bcl_2 had no statistical difference between grade Ⅰ,Ⅱ and Ⅲ. Neither did the one at various clinicopathologic stages. Over_expression of PCNA and bcl_2 and low_density of apoptotic cells are closely related to the initiation of gastric carcinoma. Over_expression of bcl_2 is related to the initiation, but is irrelated to the progression of gastric carcinoma. Over_expression of PCNA is related to both of the initiation and the progression of gastric carcinoma.
