摘 要:目的 探讨多种方法联合评价胎儿脐动脉多普勒信号的临床价值。方法 将 71例妊娠 2 0~ 41周的妇女分为正常组 ( 5 6例 )和病理妊娠组 ( 15例 )。用彩色多普勒超声测得胎儿脐动脉多普勒信号 ,将此信号以音频形式输入计算机 ,分别以零极点模型、Teagar能量法和分形方法进行信号分析 ,同时常规计算脐动脉收缩期最大血流速度 (S)与舒张期末血流速度 (D)的比值 (S/D)。以多元决策法综合上述 4种方法的特征参数评价脐动脉多普勒信号。结果 对病理妊娠组 ,利用常规S/D法、零极点模型法、Teagar能量法、分形法的异常率分别为 2 6 .6 7%、6 6 .6 7%、5 3.33%和 46 .6 7%。利用多元决策法 ,病理妊娠组异常率为 80 .0 0 %,与前 4种方法相比差异有显著性意义 (P <0 .0 5 )。结论 利用多元决策法分析脐动脉血流信号能敏感、客观、准确地反映病理状态下的脐血流改变。Objective To evaluate the clinical value of multiple methods to analyze umbilical artery Doppler signals.Methods Seventy one cases of pregnancies(gestational age 20~41 weeks) in which 56 normal and 15 abnormal were studied.Color Doppler ultrasound was used to get umbilical artery Doppler signals.The ratio of peak value of systolic flow to end stage of diastolic flow(S/D) was obtained.At the same time the audio Doppler signals were stored in computer and then to be analyzed by pole distribution,Teagar's energy algorithm method and fractal method respectively.At last above 4 methods were combined by a multi analysis method to evaluate umbilical artery Doppler signals.Results In abnormal group, 26.67% patients had S/D value > 3.0,by pole distribution method, 66.67% patients had abnormal value,by Teagar's energy algorithm method, 53.33% patients had abnormal value,by fractal method, 46.67% patients had abnormal value.By the multi analysis method,80% patients had abnormal value,significantly different from all methods described above(P< 0.05).Conclusions Multi analysis method applied to evaluate umbilical artery Doppler signals is more sensitive and comprehensive.
