摘 要:目的 探讨银杏叶提取物 (商品名达纳康 )对糖尿病视网膜病变 (diabetic retinopathy,DR)视网膜光凝术后早期视网膜功能改变的干预效果。 方法 采用前瞻性随机对照研究的方法将 6 0例 6 0只眼 ~ 期 DR患者分为达纳康用药组和对照组。应用多焦视觉诱发反应图象系统 (visual evokedresponse image system,VERIS)对视网膜激光光凝术前、术后第 3d和术后第 7d的多焦视网膜电图进行了定量研究。 结果 视网膜光凝术前对照组和用药组的年龄、性别组成、血糖、糖尿病病程和 DR病程均具有可比性。两组术前多焦视网膜电图 (multifocal electro- retinogram,MERG)各波的反应密度和 N1、P1波潜伏期差异无显著性的意义。与术前比较 ,术后第 3d用药组 MERG各波大部份象限的潜伏期无显著改变。N1波、P1波、N2波反应密度较术前减小 ,但下降的幅度均较对照组小。用药组 N1和 P1波潜伏期较对照组显著缩短。P1波和 N2波的黄斑中央 5°区域反应密度显著高于对照组。术后第 7d,两组之间 N1和 P1波潜伏期差异仍有显著性的意义 ;各波反应密度差异无显著性的意义。 结论 达纳康可能在一定程度上减轻糖尿病视网膜病变患者视网膜光凝术后 。Objective To investigate the early effects of intervention with tanakan on retinal function in diabetic retinopathy(DR) after laser photocoagulation. Methods Prospective random controlled study was performed on 60 Patients (60 eyes) from 23 to 69 years old with DR(phase Ⅲ~Ⅳ). The multifocal electroretinograms (MERG) were tested with VERIS Ⅳ before , the 3rd day and the 7th day after photocoagulation. Results No significant differences were found in the latencies and response densities of N1,P1 and N2 between the two groups before photocoagulation. Compared with that before photocoagulation, three days after photocoagulation the latencies in tanakan group had no significant change. The response densities of N1,P1 and N2 reduced and the changes were much smaller than that in control. Three days after photocoagulation ,the response densities of P1 and N2 in the central macula 5° area were much higher and the latencies of P1 and N2 were significantly shorter than that in control group. There were no significant differences in the response densities in the 7th day and the differences in the latencies between two groups still existed. Conclusion Tanakan may be effective in preventing the retina from damage of retinal photocoagulation in some degree in DR.
