超选择靶区动脉置泵综合治疗晚期胃肠道肿瘤  

Super-selective Pump-insertion into the Target Artery with Regional Infusion Chemotherapy and Immunity Agent in Treatment for Gastrointestinal Cancer

摘　　要：[目的]评价术中超选择靶区动脉内置泵,术后皮下泵内区域灌注化疗药物及免疫制剂治疗晚期胃肠道肿瘤的疗效。[方法]随访观察88例手术证实不能切除的晚期胃肠道肿瘤患者,经术中选择性肿瘤供血动脉内置泵,术后泵内灌注化疗药物及免疫制剂后的疗效。其中胃癌45例,直肠癌31例,结肠癌12例。[结果]化疗后,完全缓解（CR）2例,部分缓解（PR）77例,其中11例行二次根治性癌肿切除术;稳定者（NC）9例,无1例恶化者（PD）。有效率89.8％（79/88）。置泵术后生存满1、2、3年者分别为86.4％（76/88）、30.7％（27/88）、10.2％（9/88）,平均生存期为21.5个月。[结论]术中超选择靶区动脉内置泵,术后泵内灌注化疗药物及免疫制剂是治疗晚期胃肠道肿瘤的有效方法,可以延长患者的生存期,提高手术切除率。[Purpose] To evaluate the effect of intraoperative selective pump-insertion into the tumor target artery with postoperative regional infusion chemotherapy and immunity agent in treatment for unresectable gastrointestinal cancer. [Methods] The effect of intraoperative super-selective pump-insertion into the tumorous nutritious artery, postoperative regional infusion chemotherapy and immunity agent in 88 patients with unresectable gastrointestinal cancer was observed. Of them these were 45 cases with gastric cancer, 31 cases with rectal cancer, 11 cases with colon cancer. [ Results ] Overall response rate to chemotherapy was 89.8 % (79/88) including 2 cases with CR, 77 cases with PR, 9 cases with NC and no case with PD.Of the CBMB wilh PR, 11 cases were performed secondary radical operation.Following pump-insertion, 1-, 2-, and 3-year survival rates were 86.4% (76/88) ,30,7% (27/81) and 10.2% (9/88) respectively, with an average survival of 21.5 months, [Conclusion] Superselective pump-insertion into the artery with fBjjenal jnfnsion chemotherapy is an effective way in treatment for unreseotable gastrointestinal cancer,which may prolong the survival period of patlents and inorease the resectable rate.
