足外侧皮瓣修复足跟恶性黑色素瘤的治疗体会

摘　　要：目的　为足跟部皮肤恶性黑色素瘤彻底切除后修复提供质地优良的皮瓣。方法　根据足外侧皮瓣的应用解剖学基础 ,设计皮瓣修复足跟部皮肤恶性黑色素瘤扩大切除后创面 ,供瓣区以游离皮片移植修复。结果　 3例患者皮瓣全部存活 ,随访 3个月至 3年 ,术区外形和功能恢复良好 ,现患者均存活。结论　足外侧皮瓣具有皮肤质地好、富有弹性、有感觉、外形不臃肿、血管易解剖、转移灵活方便等优点 ;该皮瓣用于足跟部皮肤软组织缺损的修复操作方便简单 ,对足跟部支撑重物、缓冲震荡和耐磨抗压起到了十分重要的作用。足外侧皮瓣修复足跟部恶性黑色素瘤的方法是较为理想的手术方法。Objective To provide a good site of skin flap for repairing skin defect after radical resection of malignant melanoma of heel. Methods Based on the applied anatomy of skin flap at the lateral site of foot, skin flaps were devised to repair skin defects after expanded resection of malignant melanoma at the heel of foot, after following up for 3 months to 3 years, the sites providing skin flaps were then repaired by free skin graft. Results All the skin flaps of these 3 cases survived after following up for 3 month to 3 years, the outline and function were good after operation, all the patients survived. Conclusion The skin flap at the lateral site of foot has a good texture, elasticity and sensation, its blood vessels can be easily dissected out and be moved conveniently, such skin flaps can be conveniently used to repair skin defects at the heel of foot. After repair, the heel outline is good and still maintain such important functions as supporting body weight, buffering vibration and tolerating abrasion. So it′s an ideal method to repair malignant melanoma at the heel of feet.
