不同冠边缘对牙周组织影响的临床分析

摘　　要：目的:探讨全冠边缘的位置和材料对牙周组织的影响。方法:选择临床上需要全冠修复的牙齿,按肩台设计分为四组。A组:全瓷肩台,平龈缘;B组:全瓷肩台,位于龈下;C组:金瓷肩台,平龈缘;D组:金瓷肩台,位于龈下。在牙体预备前、烤瓷冠粘附4周后,分别检测并记录颊侧的菌斑指数(the plaque index,PLI)、牙周探诊深度(periodontal probe depth,PD)、龈沟出血指数(sulcus bleeding index,SBI)。结果:冠修复前后各牙周指数对比,A组和B组的PLI、GI、SBI无统计学差异(P>0.05);C组的GI和SBI无统计学差异(P>0.05);C组的PLI和D组的PLI、GI、SBI有统计学差异(P<0.05)。冠修复后,同一种肩台材料(全瓷肩台或金瓷肩台),不同肩台位置(平龈或龈下)的设计对牙周健康的影响无统计学差异(P>0.05)。对于同一种肩台位置(平龈或龈下),不同肩台材料(全瓷肩台或金瓷肩台)设计对牙周健康的影响无统计学差异(P>0.05)。结论:瓷肩台和金瓷肩台位于龈上还是龈下对牙周健康的影响没有差异,但冠修复前后的对比,发现全瓷肩台优于金瓷肩台。Objectives To evaluate the influence of the PDF crown margins on the periodontal conditions. Methods Theteeth were selected for full crown restoration, according to the shoulder design were divided into four groups. In group A, fullceramic shoulder and with gingival flush. In group B, all ceramic shoulder and subgingival, group C porcelain –metal shoulderand with gingival flush, group D Porcelain –metal shoulder in subgingival. And the plaque index (PLI), periodontal probedepth (PD) and gingival sulcus bleeding index (SBI) were measured and recorded before teeth preparation and after 4 weeks ofcementation of PFM crowns. Results After crown restoration the periodontal index contrast, group A and group B PLI, GI SBIno statistical difference (P >0.05); C group of GI and SBI no statistical difference (P >0.05); C group of PLI and group D PLI, GISBI has significant difference (P <0.05). After crown restoration, for the same kind of shoulder materials (all porcelain shoulderor porcelain-metal shoulder), the influence of different shoulder position (flat gum or gum) on periodontal health was notstatistically significant (P >0.05). There was no significant difference (P >0.05) in the design of the periodontal health of the samekind of shoulder position (flat or under the gum). Conclusion The effect of all ceramic shoulder and porcelain-metal shoulderon the periodontal health was not different. But before and after the restoration of the comparison, it was found that the wholeporcelain shoulder was superior to the porcelain-metal shoulder.
