国内进展性脑梗死危险因素的系统评价

摘　　要：目的综合评价国内进展性脑梗死患病人群的危险因素。方法采用Meta分析的方法对国内有关进展性脑梗死发病危险因素的研究结果进行定量综合分析;使用Review Manager 5.17对研究结果进行异质性检验以及OR值的合并和95%CI的计算。结果共纳入15篇文献,病例组1 345例,对照组2 643例。Meta分析结果显示:中国人群进展性脑梗死的危险因素有糖尿病(OR=3.51,95%CI:2.43~5.06,P=0.003)、高血压(OR=3.90,95%CI:1.98~7.67,P<0.000 1)、发热(OR=4.42,95%CI:2.64~7.39,P=0.01)、空腹血糖(OR=1.33,95%CI:0.77~1.90,P<0.000 01)、白细胞计数(OR=1.48,95%CI:0.28~2.69,P<0.00001)、C反应蛋白(OR=1.59,95%CI:0.34~2.84,P=0.000 3)、收缩压(OR=-20.85,95%CI:-30.18^-11.51,P<0.000 01)。结论现有的证据表明中国人群进展性脑梗死的危险因素有糖尿病、高血压、发热、空腹血糖、白细胞计数、C反应蛋白、收缩压。Objective To evaluate the risk factors of progressive cerebral infarction(PCI)in Chinese population.Methods A comprehensive quantitative analysis of the domestic research results of PCI risk factors of Chinese population was conducted by Meta analysis method.Using RevMan 5.1.7,test for heterogeneity was performed,pooled OR value and 95% confidence interval calculated.ResultsThere were totally 15 articles matching the study,including 1 345 PCI patients and 2 643 controls.The Meta analysis showed that the risk factors of PCI in Chinese population were diabetes(OR =3.51,95% CI :2.43 to 5.06,P =0.003),hypertension(OR =3.90,95% CI :1.98 to 7.67,P <0.000 1),fever(OR =4.42,95% CI :2.64 to 7.39,P =0.01),fasting blood glucose (FBG)(OR =1.33,95% CI :0.77 to 1.90,P <0.000 01),fibrinogen (FIB)(OR =1.23,95% CI :1.10 to 1.30,P =0.27),white blood cell (WBC)(OR =1.48,95% CI :0.28 to 2.69,P <0.000 01)and C reactive protein (CRP)(OR =1.59,95% CI :0.34 to 2.84,P =0.000 3),systolic blood pressure (SBP)(OR =- 20.85,95% CI :- 30.18 to - 11.51,P <0.000 01).Conclusion For Chinese population,the risk factors of PCI are diabetes,hypertension,fever,FBG,FIB,WBC,CRP,and SBP.
