社区慢性病管理与老年患者身心照顾的探讨

摘　　要：目的 调查分析2012年1月～2014年12月济南市历下区社区慢性病管理现状与老年患者身心照顾问题。方法 根据济南市历下区社区卫生服务中心登记的慢性病患者的资料进行电话回访和家庭随访，分析慢性病的组成与占比情况，以及老年慢性病患者的身心健康影响因素。结果 高血压病、糖尿病所占比例较高，分别为29.60% 和19.70%。影响患者身心健康的因素中，基础疾病和经济状况所占比例较大，分别为46.10%和22.00%。结论 关爱老年慢性病患者的身心健康，提高其生活质量是社区卫生服务中心今后努力的方向。Objective Investigation and analysis data of community chronic disease management from January 2012 to December 2014, and the problems of elderly patients’ physical and psychological care. Methods According to data of jinan lixia district community health service center registration, Telephone follow-up and family follow-up were taken to collect the information above. Results The proportion of hypertension, diabetes were Higher, were 29.60% and 19.70%, respectively. Factors affect patients' physical and mental health were basic diseases and the economy, 46.08% and 22.03%, respectively.Conclusion Mental and physical health care in elderly patients with chronic diseases were important. Improve their quality of life is the future struggle direction of community health service centers.
