胚胎角膜干细胞移植治疗翼状胬肉的远期临床观察

摘　　要：目的：探讨胚胎角膜干细胞移植治疗翼状胬肉的远期临床效果。方法：收治翼状胬肉患者100例(108眼)，随机分成观察组和对照组各50例(54眼)，对照组在显微镜下常规切除翼状胬肉头部，形成6mm×8mm大小的巩膜裸露区。将结膜间断固定于浅层巩膜，暴露宽度3～4mm，术毕包扎，48h后给予氧氟沙星眼液滴眼，第5天改用典必舒滴眼液滴眼，术后10d拆线。观察组在对照组的基础上，在结膜间断固定于浅层巩膜时，将上述准备好的移植材料，平铺于暴露巩膜的创面上，缝合，剪除多余羊膜组织。术后治疗与对照组相同。结果：观察组50例(54眼)均未见上皮排斥线，术后3d术区角膜上皮愈合，拆线后羊膜水肿消失，愈合率100.0%，经过1～36个月的随访，复发1例，复发率1.9%，对照组50例(54眼)，45眼5～7d术区角膜上皮愈合，愈合率83.3%(45/54)，经过1～36个月的随访，复发20眼，复发率37%(20/54)。结论：胚胎角膜干细胞移植治疗翼状胬肉的复发率低，远期疗效好。Objective:To explore the long term clinical observation of embryonic corneal stem cell transplantation in the treatmentof pterygium.Methods:100 patients(108 eyes) with pterygium were selected,they were randomly divided into the observation groupand the control group with 50 cases(54 eyes),the control group was treated with conventional excision of pterygium undermicroscope,scleral exposed area with 6 mm×8 mm was formed.Conjunctiva was fixed in shallow layer scleral discontinuity,thewidth of exposure was 3 to 4 mm,after surgery,the wound was bandaged,48 hours later,the patient was given levofloxacin eye drops,on the fifth day,the eye drops were changed to Dianbishu eye drops,at 10 days after operation,the wound can take out stitches.Inthe observation group,on the basis of the control group,the prepared material was placed on the wound when conjunctiva was fixedin shallow layer scleral discontinuity,and then suture the wound,excess amniotic tissue was cut off.After surgery,the treatment wassame as that of the control group.Results:In the observation group of 50 cases(54 eyes),they was no epithelial rejection line,at 3days after surgery,corneal epithelium was healed,after suture removal,amniotic membrane edema was disappeared,the healing ratewas 100%,after 1 to 36 months of follow-up,1 case was recurred,the recurrence rate was 1.9%,in the control group of 50 cases(54eyes),at 5 to 7 days later,the corneal epithelium of 45 eyes was healed,the healing rate was 83.3%(45/54),after 1 to 36 months offollow-up,20 eyes were recurrented,and the recurrence rate was 37%(20/54).Conclusion:The recurrence rate of embryonic cornealstem cell transplantation in the treatment of pterygium was low,and the long-term effect was good.
