摘  要:目的:观察新生儿缺氧缺血性脑病(HIE)颅脑多普勒超声检查(TCD)大脑中动脉(MCA)脑血流速度(CBFV)的不同变化。方法:收治HIE足月儿100例,分为轻度、中重度各50例,无HIE足月儿50例作为对照组,记录其临床表现,应用颅脑多普勒超声于其生后检查MCA各项血流参数,讨论分析HIE患儿的各项血流参数的动态变化与HIE的发生之间的相关性。结果:HIE组Vs、Vd较对照组减慢;缺氧缺血性脑病越重,Vs、Vd流速减慢越明显,RI、PI、Vs/Vd值越高,差异具有统计学意义(P<0.05)。结论:新生儿HIE患儿脑血流出现异常变化,应用TCD检查时可发现MCA各项血流参数均有改变,且具有及时、无创、准确、经济、可重复性的优势。Objective:To observe the different changes of cerebral blood flow velocity(CBFV)in the middle cerebral artery(MCA)in neonates with hypoxic ischemic encephalopathy after transcranial doppler(TCD)examination.Methods:100 cases of full-term infants with hypoxic ischemic encephalopathy(HIE) were selected.They were divided into mild,moderate and severe with each of 50 cases.50 cases of full-term infants without HIE were selected as the control group.We recorded their clinical manifestations.We used the transcranial doppler to examine the blood flow parameters of MCA after birth,to discuss the correlation between the dynamic changes of the blood flow parameters and the occurrence of HIE in children with HIE.Results:Compared with the control group,Vd and Vs in the HIE showed more slower,the more severe HIE,the more obvious slower of Vs and Vd flow rate;the higher RI,PI,Vs/Vd value;the differences were statistically significant(P<0.05).Conclusion:The cerebral blood flow changes in neonateswith HIE,TCD examination can found the parameters of the blood flow of MCA changed,and it has the advantages of timely,non-invasive,accurate,economic and repeatable.
